脂蛋白相关磷脂酶A2在冠状动脉粥样硬化性心脏病患者中的临床应用价值

摘要 目的讨论脂蛋白相关磷脂酶A2(LP-PLA2)在冠状动脉粥样硬化性心脏病(CHD)患者中的临床应用价值。方法选取江阴市人民医院2018年6月至2019年6月收治的64例CHD病患者为研究对象,按照其危险因素分为两组,伴随其他危险因素组31例(CHD2),单纯冠心病组33例(CHD1),另选取健康人员为对照组35例。检测CHD病患者及健康患者中的脂蛋白相关磷脂酶A2及HDL-C、LDL-C、TG、TC。结果CHD1组LP-PLA2数值高于对照组(P<0.05),HDL-C数值前者低于后者(P<0.05)。就危险因素,CHD2组LP-PLA2、TG、LDL-C数值较高(P<0.05);HDL-C数值低于对照组(P<0.05)。CHD2组TG、LDL-C数值高于CHD1组,差异有统计学意义(P<0.05);LP-PLA2、TG、LDL-C数值比较,差异无统计学意义(P>0.05);不同病变支数患者的LP-PLA2数值对比,差异无统计学意义(P>0.05)。对不同病变患者机体内的LP-PLA2进行单因素分析,结果表明三支病变患者LP-PLA2水平高于单支;冠状动脉粥样硬化性心脏病独立危险因素为Lp-PLA2,HDL是冠心病的保护因素。结论冠心病患者LP-PLA2水平明显升高,血浆检测诊断价值显著。 Objective To investigate the clinical value of lipoprotein associated phospholipase A2(Lp-PLA2)in patients with coronary atherosclerotic heart disease(CHD).Methods 64 patients with CHD admitted to Jiangyin People's Hospital from June 2018 to June 2019 were selected as the research objects.According to their risk factors,they were divided into two groups:31 cases with other risk factors(chd2),33 cases with simple coronary heart disease(CHD1),and 35 cases of healthy people as the control group.Lipoprotein associated phospholipase A2,HDL-C,LDL-C,TG,TC in CHD patients and healthy patients were detected.Results The value of LP-PLA2 in CHD1 group was higher than that in the control group(P<0.05),and the value of HDL-C was lower than that in the control group(P<0.05).The values of Lp-PLA2,TG and LDL-C in CHD2 group were higher than those in control group(P<0.05),HDL-C value was lower than that in control group(P<0.05),and other indexes had no significant difference(P>0.05).TG and LDL-C in CHD2 group were higher than those in CHD1 group(P<0.05),but there was no significant difference in Lp-PLA2,TG and LDL-C(P>0.05)There was no significant difference in the value of Lp-PLA2(P>0.05).The results of univariate analysis showed that the level of Lp-PLA2 in patients with three vessel disease was higher than that in patients with single vessel disease;the independent risk factor of coronary heart disease was Lp-PLA2,and HDL was the protective factor of coronary heart disease.Conclusion The level of Lp-PLA2 in patients with coronary heart disease will increase tomorrow,and the diagnostic value of plasma detection is significant.
