摘要
目的探索分析超声引导下腰丛-坐骨神经阻滞以及单侧腰麻对于老年患者单侧下肢手术的不同疗效。方法选择我院2019年1月至7月期间收治的单侧下肢手术老年患者42例为研究对象并随机分组,对照组21例均接受单侧腰麻,研究组21例均接受超声引导腰丛-坐骨神经阻滞麻醉。将二者的疗效进行对比。结果研究组患者麻醉总有效率高达95.24%,显著超过对照组(66.67%)(P <0.05)。治疗前,两组患者平均动脉压(MAP)及心率(HR)等血流动力学相关指标相近(P> 0.05);完成治疗后,研究组患者血流动力学相关指标均显著优于对照组(P <0.05)。研究组患者麻醉起效时间早于对照组(P <0.05),且麻醉维持时间长于对照组(P <0.05)。研究组患者不良反应发生率仅为4.76%,显著性低于对照组的33.33%(P <0.05)。结论超声引导腰丛-坐骨神经阻滞对于老年单侧下肢手术患者疗效确切,有助于稳定血流动力学,延长麻醉维持时间,且不良反应率低、安全性高,具有极大的推广应用价值。
Objective To explore and analyze the diff erent therapeutic eff ects of ultrasound-guided lumbar plexus-sciatic nerve block and unilateral lumbar anesthesia on unilateral lower limb surgery in elderly patients.Methods Forty-two elderly patients with unilateral lower limb surgery admitted to our hospital from January 2019 to July 2019 were selected as research objects and randomly divided into two groups.21 patients in the control group received unilateral lumbar anesthesia and 21 patients in the study group received ultrasound-guided lumbar plexus-sciatic nerve block anesthesia.The therapeutic eff ects were compared.Results The total eff ective rate of anesthesia in the study group was as high as 95.24%,signifi cantly higher than 66.67%in the control group(P<0.05).Before treatment,the hemodynamic indexes such as mean arterial pressure(MAP)and heart rate(HR)were similar between the two groups(P>0.05).After the treatment,the hemodynamic indexes in the study group were signifi cantly better than those in the control group(P<0.05).The onset of anesthesia in the study group was earlier than that in the control group(P<0.05)and the maintenance time of anesthesia was longer than that in the control group(P<0.05).The incidence of adverse reactions in the study group was only 4.76%,signifi cantly lower than 33.33%in the control group(P<0.05).Conclusion Ultrasound-guided lumbar plexus-sciatic nerve block is effective for elderly patients undergoing unilateral lower limb surgery.It is helpful to stabilize hemodynamics,prolong anesthesia maintenance time,has low adverse reaction rate and high safety,and has great popularization and application value.
