摘要
目的探讨分析TCT检查阴性的接触性出血患者宫颈筛查的临床结果。方法收集我院门诊2015年1月~2019年12月收治的200例TCT检查阴性的接触性出血患者,同时进行HPV检测、阴道镜检查,并将HPV检测、阴道镜检查、HPV联合阴道镜检查阳性的结果分别与病理检查阳性结果进行一致性、灵敏度、特异度、阳性预测值、阴性预测值比较分析。结果入选患者中HPV检测阳性73例,阴道镜检查阳性62例,宫颈病理活检阳性65例。比较分析HPV联合阴道镜检查与宫颈活检的阳性检出率具有高度的一致性(k=0.879),其灵敏度、特异度、阳性预测值、阴性预测值分别是98%、93%、86%、99%,明显高于单用HPV检测或阴道镜检查。结论对接触性出血患者,直接宫颈HPV检测联合阴道镜检查用于筛查宫颈癌及宫颈癌前病变的效能明显高于单用TCT检测、宫颈HPV检测或阴道镜检查,能有效减少误诊或漏诊,缩短患者等待周期。
Objective To explore and analyze the clinical results of cervical screening in the patients with contact bleeding who had negative TCT results.Methods From January 2015 to December 2019,200 patients with contact bleeding who had negative TCT results admitted to and treated in the outpatient service of our hospital were collected and were given HPV test and colposcopy.A comparative analysis was made on the consistency,sensitivity,specificity,positive predictive value and negative predictive value between the positive results of HPV test,colposcopy and HPV test combined with colposcopy and the positive results of pathological examination.Results Among the selected patients,there were 73 positive results of HPV test,62 positive results of colposcopy and 65 positive results of cervical pathological biopsy.The comparative analysis showed that the positive rates of HPV test combined with colposcopy and cervical biopsy were highly consistent(k=0.879),and the sensitivity,specificity,positive predictive value and negative predictive value were 98%,93%,86%and 99%respectively,which were obviously superior to those of merely HPV test or colposcopy.Conclusion For the patients with contact bleeding,the efficiency of direct cervical HPV test combined with colposcopy in screening cervical cancer and cervical precancerous lesions is obviously higher than that of merely TCT test,cervical HPV test or colposcopy.It can effectively reduce misdiagnosis or missed diagnosis and shorten the waiting period for the patients.
