摘要
目的:观察生化汤联合失笑散对初产妇产后恶露不尽患者的临床疗效。方法:将500例产后恶露不尽初产妇随机分为对照组244例和研究组256例。在常规治疗基础上,对照组予以缩宫素注射液20U,每天1次,肌肉注射;研究组予以生化汤联合失笑散100ml,每天2次,黄酒送服。2组患者均治疗7天。比较2组间临床疗效、症状体征改善情况、子宫恢复情况、血液流变学指标及不良反应发生率。结果:对照组有效率(82.38%)低于研究组有效率(94.91%),具有统计学意义(P<0.05)。与对照组比较,研究组治疗后小腹痛缓解时间、子宫压痛缓解时间、腰腹重坠消失时间、恶露消失时间较短,阴道出血量较低,治疗后子宫体积及子宫积液量较低,治疗后全血高切粘度、全血低切粘度、血浆粘度以及红细胞聚集指数较低,具有统计学意义(P<0.05)。治疗过程中,对照组不良反应为恶心呕吐、血压下降,研究组不良反应为恶心呕吐、腹胀、食欲不振。研究组不良反应发生率为4.29%,对照组不良反应发生率为4.93%,两组间差异无统计学意义(P>0.05)。结论:生化汤联合失笑散治疗初产妇产后恶露不尽患者的临床疗效确切,能明显加速临床症状及体征消失,促进子宫缩复,减少子宫积液,改善血液流变学指标,且具有较高安全性。
Objective:To investigate the curative effect of Shenghua Decotioin combined with Shixiao Powder in primipara with postpartum lochiorrhea.Methods:500 primipara with postpartum lochiorrhea were randomly divided into controlled group with 244 cases and experiment group with 256 cases.On the basis of routine treatment,the controlled group treatment with Oxytocin Injection 20U,qd,intramuscular injection.The experiment group received Shenghua Decoction combined with Shixiao Powder 100 ml,bid,taken with yellow wine.2 groups of patients were treated for 7d.The clinical efficacy,improvement of symptoms and signs,uterine recovery,hemorheological indexes and incidence of adverse reactions were compared between 2 grops.Results:The effective rate in the controlleed group(82.38%)was lower than that in the experiment group(94.91%),with significant difference(P<0.05).Compared with the controlled group,the small abdominal pain and uterine tenderness relief time,waist and abdomen heavy fall loss time,lochia disappear time were shorter,the amount of vaginal bleeding was lower after treatment in the experiment group,the volume of uterus and the amount of hydrops in the uterus were lower after treatment,the whole blood high shear viscosity,whole blood low shear viscosity,plasma viscosity and erythrocyte aggregation index were lower after treatment,with significant difference(P<0.05).In the course of treatment,the adverse reactions in the controlled group were nausea and vomiting,blood pressure drop,the adverse reactions in the experiment group were nausea and vomiting,bdominal distention and loss of appetite.The incidence of adverse reactions in the experiment group was 4.29%,and the incidence of adverse reactions in the controlled group was 4.93%,there was no significant difference between the two groups(P>0.05).Conclusion:The curative effect of Shenghua Decoction combined with Shixiao Powder in the treatment of primipara with postpartum lochiorrhea is accurate,it can obviously accelerate the disappearance of clinical symptoms and s
