摘要
目的研究孕三烯酮联合亮丙瑞林治疗子宫内膜异位症的效果及对抗心磷脂抗体(ACA)的水平影响。方法选择行腹腔镜手术的58例子宫内膜异位症患者,随机分为对照组与观察组。对照组术后单纯口服孕三烯酮,观察组联合皮下注射亮丙瑞林。比较2组的痛经、盆腔痛、性交痛积分和细胞因子水平的改变情况。结果观察组总有效率明显高于对照组(P<0.05);治疗后,2组血清ACA的阳性率均明显降低,且观察组降低更明显(P<0.05);治疗后,2组痛经、盆腔痛及性交痛积分均明显降低,且观察组降低更明显(P<0.05);观察组术后妊娠成功率明显高于对照组(P<0.05),观察组术后1年的复发率明显低于对照组(P<0.05)。结论孕三烯酮联合亮丙瑞林能明显提高子宫内膜异位症患者的临床疗效,降低自身免疫抗体阳性率,改善临床结局。
Objective To study the effect of gestrinone combined with leuprorelin in the treatment of endometriosis.Methods A total of 58 cases of patients with endometriosis were divided into two groups randomly.The control group was given gestrinone only after operation,while the observation group was given leuprorelin subcutaneously.The changes of dysmenorrhea,pelvic pain,sexual pain score and cytokine levels were compared between the two groups.Results The effective rate in the observation group was significantly higher than that in the control group(P<0.05).After treatment,the positive rate of serum ACA in both groups were significantly decreased,and the observation group was significantly lower(P<0.05).After treatment,the scores of dysmenorrhea,pelvic pain and sexual intercourse pain in the two groups were significantly reduced,and those in the observation group were significantly lower(P<0.05).The pregnancy success rate in the observation group was significantly higher than that in the control group(P<0.05),and the recurrence rate in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Progesterone combined with leuprorelin can significantly improve the efficacy of endometriosis patients,reduce positive rate of serum ACA,and improve clinical outcomes.
