摘要
早期痿证,病情多轻浅,治疗上多选用手足阳明经穴进行治疗。痿证日久,伤及脏腑,病势缠绵,故治疗上以"主取阳明"为法,在调护阳明经脉气血的同时,兼顾他经他脏。对于病情复杂,病变范围广、疾病所涉经脉较多的痿证,不仅治疗上重点选择阳明经,并酌情选用手足三阴经、手足太阳经、手足少阳经施治,此谓之"多经多穴"法。在选阳明经穴的基础上,同时选用督脉以及八会穴、原穴等有特殊作用的穴位。电针治疗老年肌少症的作用机制有:改善低炎症状态,调节激素水平,影响细胞自噬与凋亡,调节泛素-蛋白酶系统。老年肌少症病因病机复杂,病机关键在于肌肉瘦消无力,筋脉痿弱不用,故取穴总不离阳明经。电针治疗老年肌少症,阳明经足三里穴是治疗中的基础穴位,也是必选穴位,在此基础上加用足少阳经环跳穴、足太阳经承山穴等。
In terms of early flaccidity syndrome,the disease is much light and shallow,the points of hand and foot Yangming meridians are often selected for treatment.Long-time flaccidity syndrome for a long time hurts the viscera,the disease will be lingering,so the treatment focuses on"taking Yangming meridian",regulating and protecting qi and blood of Yangming meridians,and other meridians and organs should also be taken into account.For the flaccidity syndrome with complex condition,wide range of lesions and more channels involved in the disease,not only the Yangming meridians should be selected as the treatment focus,but also the three yin meridians of hands and feet,the Taiyang meridian of hands and feet,and the Shaoyang meridian of hands and feet should be selected as appropriate,which is called method of"multiple channels and multiple points".On the basis of selecting points of Yangming meridians,some special points such as GV,eight meeting points,source points,etc.were selected at the same time.The mechanism of electroacupuncture in the treatment of senile sarcopenia includes:improving the state of low inflammation,regulating hormone level,affecting autophagy and apoptosis,and regulating ubiquitin-protease system.The etiology and pathogenesis of senile sarcopenia are complex.The key to the pathogenesis is that the muscles are thin and weak,and the tendons and veins are flaccid.Therefore,the points are always selected from the Yangming meridians.In the treatment of senile sarcopenia with electroacupuncture,Zusanli(ST 36)of Yangming meridian is the basic and necessarypoint.On this basis,Huantiao(GB 30)of foot Shaoyang meridian and Chengshan(BL 57)of foot Taiyang meridian can also be added.
