摘要
目的探讨原发性高血压患者动态血压负荷与中性粒细胞与淋巴细胞计数比值(NLR)及高敏C反应蛋白(hs-CRP)水平的关系及临床意义。方法选择2017年1月~2018年3月我院心内科住院及老年医学科原发性高血压病患者70例作为高血压组,另选择同期门诊健康体查者34名作为对照组;检测两组24 h动态血压、动态血压负荷水平、血常规、hsCRP、血糖、血脂及肝肾功能等生化指标,分析动态血压负荷与NLR及hs-CRP的关系。结果①高血压组NLR及hs-CRP水平高于对照组,差异有统计学意义(P<0.05);②高血压组24 h血压负荷、白天及夜间血压负荷高于对照组,差异具有统计学意义(P<0.05);③相关性分析显示,NLR及hs-CRP与24 h收缩压负荷(r=0.495、r=0.391)、24 h舒张压负荷(r=0.522、r=0.187)、日间收缩压负荷(r=0.536、r=0.369)、日间舒张压负荷(r=0.526、r=0.366)、夜间收缩压负荷(r=0.526、r=0.361)、夜间舒张压负荷(r=0.533、r=0.397)均呈正相关;④多元线性回归分析显示,年龄、NLR是24 h收缩压负荷的独立影响因素。结论NLR及hsCRP在高血压病患者中显著增高,动态血压负荷与NLR、hsCRP之间存在正相关,炎症机制对高血压病发病及进展可能存在影响。
Objective To explore the relationship and clinical significance of ambulatory blood pressure load,neutrophil to lymphocyte count ratio(NLR)and high sensitivity C-reactive protein(hs-CRP)levels in patients with essential hypertension.Methods From January 2017 to March 2018,70 patients with primary hypertension in the cardiology department and geriatrics department of our hospital were selected as the hypertension group,and 34 outpatient health examinations during the same period were selected as the control group;two detections group 24 h ambulatory blood pressure,ambulatory blood pressure load level,blood routine,hsCRP,blood glucose,blood lipids and liver and kidney function and other biochemical indicators,analyze the relationship between ambulatory blood pressure load and NLR and hs-CRP.Results①The levels of NLR and hs-CRP in the hypertension group were higher than those in the control group,the difference was statistically significant(P<0.05);②The 24h blood pressure load,day and night blood pressure load in the hypertension group were higher than those in the control group,the difference was statistically significant(P<0.05);③Correlation analysis showed that NLR and hs-CRP were associated with 24h systolic pressure load(r=0.495,r=0.391),24h diastolic pressure load(r=0.522,r=0.187),and day contraction compressive load(r=0.536,r=0.369),day diastolic pressure load(r=0.526,r=0.366),night systolic pressure load(r=0.526,r=0.361),night diastolic pressure load(r=0.533,r=0.397)were positively correlated;④Multiple linear regression analysis showed that age and NLR were independent influencing factors of systolic blood pressure load at 24 h.Conclusion NLR and hsCRP were significantly increased in patients with hypertension.There was a positive correlation between ambulatory blood pressure load and NLR and hsCRP.The inflammatory mechanism might have an impact on the incidence and progression of hypertension.
