摘要
目的评价CT诊断支原体肺炎的临床价值。方法回顾性分析2016年4月至2019年5月沈阳市儿童医院收治的90例疑似支原体肺炎患儿的临床资料,均行CT检查和X线检查,以症状、体征、影像学、必要的病原菌培养等综合诊断结果为金标准,比较X线检查和CT检查诊断支原体肺炎的检出率、准确率、灵敏度、特异度。结果CT检查的影像学特征主要表现为肺部实质阴影、纵隔淋巴结肿大,左侧胸腔有少许积液;CT检查支原体肺炎的检出率为97.53%,X线检查的检出率为88.89%,差异有统计学意义(P<0.05);CT检查确诊单侧、双侧肺叶发生病变的准确率分别为97.5%、97.56%,X线检查分别为85.00%、85.37%,差异有统计学意义(P<0.05)。CT扫描检查结果确诊肺炎支原体肺炎灵敏度、特异度分别是94.94%、94.94%,明显高于X线检查的87.5%、86.11%。结论CT检查可提高支原体肺炎的检出率,且可清晰显示单侧及双侧肺叶的病变征象。
Objective To evaluate CT in the diagnosis of pediatric mycoplasma pneumoniae pneumonia in the clinical effect.Methods From April 2016 to December 2019,90 children with mycoplasma pneumoniae pneumonia admitted to our hospital were selected as the study objects.The imaging features of CT scan and X-ray were analyzed retrospectively.According to the symptoms,signs,imaging,necessary pathogen culture and other comprehensive diagnostic results as the gold standard.The accuracy of X-ray and CT diagnosis of mycoplasma pneumoniae pneumonia-related signs in children were compared.Results The imaging features of CT scans were mainly parenchymal shadows of the lungs,enlarged mediastinal lymph nodes,and a little effusion in the left thoracic cavity.The positive rate of CT scans was 97.53%,which was significantly higher than the positive rate of X-rays(P<0.05).The accuracy of diagnosis of unilateral lung lobe lesions and bilateral unilateral lobe lesions by CT scan results were 97.5%and 97.56%,which were significantly higher than the accuracy rate of X-ray scans 85.00%and 85.37%(P<0.05).The sensitivity and specificity of CT scan to diagnose Mycoplasma pneumoniae pneumonia were 94.94%and 94.94%,which were significantly higher than the 87.5%and 86.11%of X-ray examination(χ2=5.2710,6.3280,both P<0.05).Conclusion CT scan can improve the detection rate of mycoplasma pneumoniae pneumonia in children,and can clearly and intuitively show the signs of lesions in unilateral and bilateral lung lobes.
