化疗对乳腺癌患者血脂、血糖、尿酸的影响及因素分析 被引量:6

摘要 目的研究化疗对乳腺癌患者血脂、血糖、尿酸水平的影响及相关因素,为乳腺癌患者的综合管理提供诊疗依据。方法收集2014年1月~2015年12月徐州医科大学附属医院甲乳外科395例完成6~8周期规范化化疗的乳腺癌患者的血脂、血糖、尿酸及临床病理资料,分析患者接受化疗后的血脂、血糖、尿酸水平变化及相关影响因素。结果接受化疗后乳腺癌患者血清总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、空腹血糖(FBG)、尿酸(UA)水平显著高于术前,高密度脂蛋白胆固醇(HDL-C)水平显著低于术前,差异有统计学意义(P均<0.05)。单因素分析提示是否绝经(χ2=9.684,P=0.002)是患者化疗后TC水平升高的影响因素;是否绝经(χ2=17.022,P=0.000)、体重指数(BMI)(χ2=11.977,P=0.001)是患者化疗后LDL-C水平升高的影响因素;UA的升高与患者年龄(χ2=4.277,P=0.039)、是否绝经(χ2=17.944,P=0.000)、BMI(χ2=7.224,P=0.007)有关,而肿瘤分期、分子分型、化疗方案与各代谢指标变化无关。Logistic多因素分析显示,未绝经(OR=2.241,P=0.000)、BMI≤28kg/m 2(OR=2.326,P=0.007)是影响乳腺癌患者化疗后LDL-C水平升高的独立危险因素;未绝经(OR=2.663,P=0.001)是影响乳腺癌患者化疗后UA升高的独立危险因素。结论乳腺癌患者化疗后TC、TG、LDL-C、FBG、UA水平明显升高,HDL-C水平明显降低,未绝经与已绝经患者TC、LDL-C、UA升高比例不同,需定期检测血脂、血糖、尿酸的水平并及时处理,综合管理乳腺癌患者。 Objective To study the effect of chemotherapy on blood lipid,blood glucose and uric acid level in patients with breast cancer and its related factors,so as to provide the basis for the comprehensive management of breast cancer patients.Methods Totally 395 cases of breast cancer patients who completed 6-8 cycles of standardized chemotherapy from December 2014 to December 2015 were collected to analyze the changes of blood lipid,blood glucose and uric acid levels after chemotherapy and the related influencing factors.Results The levels of serum total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),fasting blood glucose(FBG)and uric acid(UA)in patients with breast cancer after chemotherapy were significantly higher than those before chemotherapy,while the levels of high density lipoprotein cholesterol(HDL-C)were significantly lower than those before chemotherapy.Univariate analysis showed that menopause(χ2=9.684,P=0.002)was the related factor of the increase of serum TC level after chemotherapy;Menopause(χ2=17.022,P=0.000),body mass index(BMI)(χ2=11.977,P=0.001)was related to the increase of serum LDL-C level after chemotherapy.The increase of UA was related to the age(χ2=4.277,P=0.039),menopause(χ2=17.944,P=0.000)and BMI(χ2=7.224,P=0.007),but tumor stage,molecular typing and chemotherapy regimen were not related to the changes of metabolic indexes.Logistic multivariate analysis showed that premenopausal(OR=2.241,P=0.000),BMI≤28kg/m 2(OR=2.326,P=0.007)were independent risk factors for the increase of LDL-C level.Premenopausal(OR=2.663,P=0.001)were independent risk factors for the increase of UA.Conclusion The levels of TC,TG,LDL-C,FBG,and UA in breast cancer patients are significantly increased after chemotherapy,and the levels of HDL-C are significantly reduced.The proportion of TC,LDL-C,and UA in premenopausal and postmenopausal patients is different.The level of blood lipids,FBG and UA should be regularly checked and dealt with in time to comprehensively manage breast cancer pa
