摘要
目的建立掌指关节的数字化三维模型,测量相关解剖学数据并进行统计分析,为掌指关节假体的设计提供部分依据。方法收集解放军920医院2017年1月~2019年12月行手部CT扫描的成人影像资料,排除掌指关节周围骨折、畸形等疾病50例,随机抽取10例为研究对象。利用Mimics、3-Matic等软件通过数字化技术对掌骨头及近节指骨基底弧面进行球面拟合,找到曲率半径及弧长。各组内数据比较应用SPSS统计软件的单因素方差分析。结果各组内行同一类数据比较分析,结果为:掌骨头正中矢状面曲率半径(3)>(2)>(1)>(4)>(5),差异有统计学意义(P<0.05);近节指骨基底正中矢状面曲率半径(1)>(2)>(4)>(5)>(3),差异无统计学意义(P﹥0.05)。结论人工掌指关节假体的设计:各指掌骨头假体关节面曲率应设计不同尺寸,而近节指骨基底关节面可设计成曲率一致的假体。
Objective To provide some basis for the design of metacarpophalangeal joint prosthesis by establishing a digital 3 D model of the metacarpophalangeal joint,measuring related anatomical data,and performing a statistical analysis.Methods The imaging data of adults who received hand CT scan in the Hospital 920,Joint Logistic Support Force of PLA from January 2017 to December 2019 were collected,50 cases with diseases such as fractures and deformities around the metacarpophalangeal joint were excluded,and then 10 cases were randomly selected from the remainder as research subjects.Mimics,3-Matic and other software were used to perform spherical fitting on the cambered surface of the base of the metacarpal head and proximal phalanx through digital technique,so as to obtain the radius of curvature and arc length.One-way analysis of variance(ANOVA)was used to compare data of each group using SPSS statistical software.Results The results of comparative analysis on the same type of data of each group were as follows:the radius of curvature of the metacarpal head in the median sagittal plane:(3)>(2)>(1)>(4)>(5)("3"referred to the third metacarpal head,and so on),the difference was statistically significant(P<0.05);the radius of curvature of the base of the proximal phalanx in the median sagittal plane:(1)>(2)>(4)>(5)>(3),the difference was not statistically significant(P>0.05).Conclusion When designing the metacarpophalangeal joint prosthesis,the metacarpal head prosthesis should be designed with a different curve of the artificial surface,while the base of the proximal phalanx should be designed with the same curve of the artificial surface.
