摘要
目的探讨学龄前期语言发育迟缓儿童语言发育状况及家长对儿童语言水平的影响。方法回顾性分析驻马店市第一人民医院2017年1月至2019年1月因语言发育迟缓就医的学龄前儿童65例,将其纳入观察组;另选取同期65例健康儿童纳入对照组。自制调查问卷收集两组儿童的一般资料。采用丹佛发育筛查测验(DDST)及格塞尔发育诊断量表(GDS)检测受试儿童语言理解与表达能力。采用GDS鉴定受试儿童行为模式,以发育年龄表示,再与实际年龄进行比较。结果对照组受试儿童适应性、大运动、精细运动、语言、个人-社交DQ值>86。观察组儿童适应性、大运动DQ在正常范围;精细动作、语言及个人-社交行为DQ值<85,其中语言功能最低。观察组适应性、大运动、精细运动、语言、个人-社交DQ值低于对照组(P<0.05)。65例语言迟缓患儿中14例为单纯语言迟缓,51例合并其他问题。合并其他问题患儿各功能区DQ值均低于单纯语言问题(P均<0.05)。仅电话随访与同龄儿童相当,能理解冷、累、饿,分清4个介词,识别4种颜色及可数1~5的发生率高于来院随访(P<0.05)。父母学历与单纯语言问题与合并其他问题患儿无相关性(P>0.05)。话迟缓家族病史、首次发现问题者、首次发现时间、就诊延迟时间影响单纯语言问题与合并其他问题患儿语言发育(P<0.05)。结论学龄前期语言发育迟缓儿童语言发育状况及家长对儿童语言水平有相关性,语言迟缓儿童的适应性、大运动、精细运动、语言、个人-社交较差。家属不能正确认识儿童语言迟缓问题,不利于早发现及早干预。
Objective To investigate the language development situation of children in preschool-aged time with language developmental delay,and influence of their parents on language skill of children.Methods The clinical data from 65 cases preschool-aged children with language developmental delay treated in Zhumadian First People’s Hospital from January 2017 to January 2019 were retrospectively analyzed,and they were selected as the observation group.And 65 cases healthy children at the same period were selected as the control group.A self-made questionnaire was used to collect the general information of the two groups.The Denver Developmental Screening Test(DDST)and Gesell Developmental Scale(GDS)was used to test the children’s ability of language comprehension and expression.GDS was also used to identify the behavior pattern of the children expressed in the developmental age,and then it was compared with that in the actual age.Results The development quotient(DQ)values of different domains including adaptation,gross motor,fine motor,language,and personal social intercourse of the control group were>86.In the observation group,the DQ values of adaptability and gross motor were in the normal ranges;the DQ values of fine motor,language and personal social intercourse were less than 85,and the language function was the lowest.The DQ values of different domains of the observation group were lower than those of the control group(P<0.05).Among the 65 children with language developmental delay,14 cases were simple language retardation and 51 cases were complicated with other problems.DQ values of each functional area in children complicated with other problems were lower than those in children with simple language problems(all P<0.05).The incidence of equivalent capacity make a distinction between cold,tired,hungry,four directions,four colors and counting 1 to 5 of children followed up by telephone was comparable to that of normal children with similar age,higher than that of children followed up by hospital visit(P<0.05).Th
