非球面切削FS-LASIK手术治疗中低度近视术后角膜Q值的变化

Changes of corneal Q value in patients with moderate to low myopia after aspherical FS-LASIK surgery

摘要 目的分析比较FS-LASIK联合非球面切削手术与普通切削术后角膜Q值的变化。方法前瞻性队列研究。随机选取同期于本院眼科激光中心行飞秒准分子激光手术矫正近视的患者60例,按照手术方式分为非球面切削组(30人,60眼)和普通切削组(30人,60眼),所有病例近视等效球镜度数≤-6.00D。分别于术前、术后1个月、术后3个月测量所有患者的裸眼视力、屈光度和角膜Q值以及角膜不规则指数(ISV)(index of surface variance)。统计分析使用SPSS19进行独立样本t检验和配对样本t检验。结果术后3个月,两组间裸眼视力(UCVA)和最佳矫正视力(BCVA)差异无统计学意义。所有患者术后BCVA均达到超过术前水平。术后两组之间等效球镜度、柱镜度差异均无统计学意义。两组术后角膜表面变异指数ISV比较差异无统计学意义;两组术后ISV均较术前增加(t=9.89、10.95,P<0.001)。两组术后角膜Q值比较差异无统计学意义;两组术后角膜Q值均较术前增加(t=17、19,P<0.001)。相关性分析结果表明,两组患者在术后3个月时,术后角膜Q值的改变量△Q与术前等效球镜呈负相关(r=-0.65、-0.79,P<0.05)。结论非球面切削与普通切削模式在FS-LASIK手术中具有同样的安全性和有效性,两种手术对于中低度近视角膜形态的影响具有一致性。 Objective:To compare the variation of cornea Q-value after receiving aspheric FS-LASIK or FS-LASIK for a treatment of myopia.Methods Prospective cohort study.60 patients(120 eyes)who underwent LASIK in our hospital were chosen to participate in current study.Patients were divided into two groups,aspheric LASIK group(30 people,60 eyes)and conventional LASIK group(30 people,60 eyes).In all cases,the myopia equivalent spherical degree was≤-6.00 D.The naked eye visual acuity,diopter and corneal Q values,and the index of surface variance(ISV)were measured before surgery,1 month after surgery,and 3 months after surgery.Statistical analysis SPSS19 was used for independent sample t-test and paired sample t-test.Results At 3 months after surgery,there was no significant difference in visual acuity(UCVA)and best corrected visual acuity(BCVA)between the two groups.No eyes lost a line of BCVA in either group.There was no significant difference in the cylinder refraction or spherical equivalent(SE)between the two groups.There was no significant difference in the ISV between the two groups.The postoperative ISV of the two groups increased compared with the preoperative(t=9.89,10.95,P<0.001).There was no significant difference in corneal Q value between the two groups.The corneal Q values of the two groups were increased compared with preoperative(t=17,19,P<0.001).Correlation analysis showed that the change of corneal Q value△Q was negatively correlated with preoperative spherical equivalent(SE)at 3 months after operation(r=-0.65,-0.79,P<0.05).Conclusions Aspherical ablation profile and standard ablation profile have the same safety and effectiveness in FS-LASIK surgery.Both ablation profile have the same effect on the asphericity of moderate and low myopia.
