摘要
目的前瞻性研究社区多病共存患者全科医疗照顾模式的效果。方法纳入该社区多病共存患者100例,2014年11月—2015年4月进行常规医疗照顾模式,2015年5—10月进行全科医疗照顾模式,同期入选邻近社区多病共存患者100例,2014年11月—2015年10月均进行常规医疗照顾模式,比较该社区多病共存患者两个不同时间段的疾病控制情况、建档情况;比较该社区与邻近社区的血压、血糖控制达标情况、慢性病知识掌握情况、建档情况。结果在控制血糖达标情况、控制血压达标情况、掌握慢性病相关知识合格率、高血压登记率、糖尿病登记率、建档情况方面该社区2015年5—10月显著优于2014年11月—2015年4月,差异有统计学意义(P<0.05)。结论社区应用全科医疗照顾模式能够有效改善多病共存患者生存质量,效果显著。
Objective To prospectively study the effect of general medical care for patients with multiple diseases in the community.Methods A total of 100 patients with coexistence of multiple diseases in this community were enrolled.Routine medical care was performed from November 2014 to April 2015,and general medical care was performed from May 2015 to October 2015.At the same time,100 patients with multiple diseases coexisting in neighboring communities were selected.Routine medical care was carried out from November 2014 to October 2015.The disease control and file establishment of patients with coexisting multiple diseases in the community were compared at two different time periods;the blood pressure and blood glucose of the community and neighboring communities were compared.Control the status of compliance,the status of chronic disease knowledge,and the status of file establishment.Results It is convenient for the community to control blood glucose compliance,control blood pressure compliance,grasp chronic disease-related knowledge,pass rate,hypertension registration rate,diabetes registration rate,and file establishment.The community from May 2015 to October 2015 was significantly better than November 2014 to April 2015,the difference was statistically significant(P<0.05).Conclusion The community-based general medical care model can effectively improve the quality of life of patients with coexisting multiple diseases,and the effect is significant.
