摘要
目的:总结多学科协作治疗难治性食管异物的诊疗体会,分析其优势,以期为该类患者的诊疗提供经验借鉴。方法:回顾性分析2017年6月-2019年3月天津医科大学总医院心胸外科收治的56例难治性食管异物患者的临床资料,将患者按照治疗方法分为A组(n=25)和B组(n=31)。A组行多学科协作治疗,B组未行多学科协作治疗。比较两组患者的异物嵌顿时间、异物取出后进食时间、住院时间、并发症发生情况。结果:两组的异物嵌顿时间和术后进食时间比较,差异均有统计学意义(P<0.05)。B组的脓肿形成或脓毒血症发生率高于A组,差异有统计学意义(P<0.05)。结论:对于难治性食管异物患者,采取多学科协作诊疗模式,能有效减少食管异物嵌顿的时间及严重并发症的发生,促进患者恢复,提高诊疗效果。
Objective:To summarize the experience of multidisciplinary collaboration in the treatment of refractory esophageal foreign body and analyze its advantages in order to provide experience for the diagnosis and treatment of this kind of patients.Method:The clinical data of 56 patients with refractory esophageal foreign body admitted to the department of Cardiothoracic Surgery,General Hospital of Tianjin Medical University from June 2017 to March 2019 were retrospectively analyzed.According to the treatment methods,the patients were divided into group A (n=25) and group B (n=31).Group A received multidisciplinary cooperative therapy,while group B received no multidisciplinary cooperative therapy.The incarceration time,feeding time,hospital stay time and complications were compared between the two groups.Result:Comparison of incarceration time and postoperative feeding time of foreign bodies between the two groups showed statistically significant differences (P<0.05).The incidence of abscess formation or sepsis in group B was higher than that in group A,the difference was statistically significant (P<0.05).Conclusion:For patients with refractory esophageal foreign body,multidisciplinary collaborative diagnosis and treatment can effectively reduce the time of esophageal foreign body incarceration and the occurrence of serious complications,promote patient recovery,and improve the diagnosis and treatment effect.
