摘要
目的探讨动态心电图在高血压合并左心室肥厚患者中的应用效果。方法选取本院2016年10月~2018年10月高血压合并左心室肥厚患者113例为观察组,同期高血压无左心室肥厚患者113例为对照组,均行生化指标检测(舒张压、收缩压、同型半胱氨酸、心率)和动态心电图检查(室间隔厚度、左室后壁厚度、左室重量指数、Sokolow-Lyons电压指数、Cornell电压指数、RI+SⅢ振幅、RVI+SV5振幅、所有导联QRS波群振幅之和)。比较两组患者检出疾病情况(房性心律失常、复杂房性心律失常、室性心律失常、复杂室性心律失常)。结果观察组患者舒张压、收缩压、同型半胱氨酸、心率、室间隔厚度、左室后壁厚度、左室重量指数、Sokolow-Lyons电压指数、Cornell电压指数、房性心律失常检出率、复杂房性心律失常检出率、室性心律失常检出率、复杂室性心律失常检出率均高于对照组(P <0.05)。观察组患者RI+SⅢ振幅、RVI+SV5振幅、所有导联QRS波群振幅之和均低于对照组(P <0.05)。结论动态心电图是高血压合并左心室肥厚患者的有效评价手段,具有较高的诊断价值。
Objective To investigate the application effect of dynamic ECG in hypertensive patients with left ventricular hypertrophy.Methods A total of 113 hypertensive patients with left ventricular hypertrophy at our hospital from October 2016 to October 2018 were selected as the observation group,while 113 hypertensive patients without left ventricular hypertrophy in the same period were selected as the control group.The two groups of patients were tested for biochemical indexes(diastolic blood pressure,systolic blood pressure,homocysteine,and heart rate)and dynamic ECG(interventricular septal thickness,left ventricular posterior wall thickness,left ventricular mass index,Sokolow-Lyons voltage index,Cornell voltage index,RI+SⅢamplitude,RVI+SV5 amplitude,and sum of all lead QRS wave group amplitudes).The detected diseases(atrial arrhythmia,complex atrial arrhythmia,ventricular arrhythmia,and complex ventricular arrhythmia)in the two groups of patients were compared.Results The diastolic blood pressure,systolic blood pressure,homocysteine,heart rate,interventricular septal thickness,left ventricular posterior wall thickness,left ventricular mass index,Sokolow-Lyons voltage index,Cornell voltage index,detection rate of atrial arrhythmia,detection rate of complex atrial arrhythmia,detection rate of ventricular arrhythmia and detection rate of complex ventricular arrhythmia in the observation group of patients were higher than those in the control group(P<0.05).The RI+SⅢamplitude,RVI+SV5 amplitude and sum of all lead QRS wave group amplitudes in the observation group of patients were lower than those in the control group(P<0.05).Conclusion Dynamic ECG is an effective evaluation method for hypertensive patients with left ventricular hypertrophy,and has higher diagnostic value.
