椎弓根钉棒联合全椎板切除术治疗无骨折脱位型颈髓损伤

摘要 目的探讨椎弓根钉棒联合全椎板切除术治疗无骨折脱位型颈髓损伤的临床疗效。方法采用椎弓根钉棒联合全椎板切除术治疗35例无骨折脱位型颈髓损伤患者。记录手术时间、术中出血量、术后并发症、术前及术后1年ASIA神经功能分级。结果手术时间为100~130(109.03±8.65)min;术中出血量为100~600(310.15±79.65)ml。35例患者均顺利完成手术。术后2例出现伤口并发症,1例出现脑脊液漏。术后1年无颈椎后凸畸形形成。患者均获得随访,时间12~48个月。术后1年ASIA神经功能分级均较术前明显改善(P<0.05)。结论椎弓根钉棒联合全椎板切除术治疗无骨折脱位型颈髓损伤临床效果满意,利于颈脊髓功能恢复。 Objective To investigate the clinical effects of cervical vertebral pedicle screw-rod internal fixation combined total laminectomy for treatment of cervical spinal cord injuries without fracture and dislocation.Methods A total of 35 patients with cervical spinal cord injuries without fracture and dislocation were treated with cervical vertebral pedicle screw-rod internal fixation combined total laminectomy.The operation time,intraoprative blood loss,postoperative complications,the ASIA nerve functional classfication at preoperation and postoperative 1 year were recorded.Results Operation time was 100~130(109.03±8.65)min.Intraoperative blood loss volume was 100~600(310.15±79.65)ml.All patients underwent surgery successfully.The 2 cases had incisium infection,1 case had cerebrospinal fluid leakage.No cervical kyphosis deformity was found during follow-up of 12~48 months.The ASIA nerve functional classification were significantly improved than the preoperation(P<0.05).Conclusions Cervical vertebral pedicle screw-rod internal fixation combined total laminectomy are effective method in the cervical spinal cord injury without fracture and dislocation,this surgery is beneficial for neurologic function recovery.
