摘要
目的探讨眩晕病例流行病学特点。方法回顾性分析2018年9月-2020年3月京东中美医院耳鼻咽喉头颈外科住院治疗的眩晕患者临床流行病学特点。结果共收治199例眩晕患者,其中男性80例(40.2%),女119例(59.8%);年龄11-80岁,平均(48.7±15.3)岁,60-69岁者所占比例最大(22.6%),其次为30-39岁(21.6%)和50-59岁(20.1%)。眩晕病例中,前庭外周性眩晕162例(81.4%),包括良性阵发性位置性眩晕(benign paroxysmal positional vertigo,BPPV)68例(34.2%)、梅尼埃病(Meniere disease,MD)27例(13.6%)、前庭神经炎(vestibular neuritis,VN)17例(8.5%)、突发性聋(sudden hearing loss,SHL)伴眩晕31例(15.6%)、良性复发性眩晕(benign recurrent vertigo,BRV)14例(7.0%)等;前庭中枢性眩晕18例(9.1%),包括前庭性偏头痛(vestibular migraine,VM)11例(5.5%)等;非前庭性眩晕8例(4.0%);无确切疾病诊断者11例(5.5%)。结论住院眩晕患者中女性多于男性,60-69岁者占比最大;前庭外周性眩晕、前庭中枢性眩晕和非前庭性眩晕分别占81%、9%和4%,约6%的患者未能获得确切的疾病诊断;BPPV、MD、VN、SHL伴眩晕、BRV为5种常见的眩晕疾病,占眩晕的78.9%。
Objective To discuss epidemiological characteristics of hospitalized patients with vertigo.Methods The data of the patients with vertigo between September 2018 and March 2020 were reviewed and the epidemiological characteristics were analyzed.Results A total of 199 hospitalized patients with vertigo were enrolled.There were 80 males(40.2%)and 119 females(59.8%).The average age was(48.7±15.3)years old.The patients aged 60-69 years accounted for 22.6%,followed by 21.6%for those aged 30-39 years and 20.1%for those aged 50-59 years.The number of diagnoses was 199,of them 162(81.4%)cases for peripheral vestibular vertigo,including 68(34.2%)cases for benign paroxysmal positional vertigo,27(13.6%)cases for Meniere disease,17(8.5%)cases for vestibular neuritis,31(15.6%)cases for sudden hearing loss with vertigo,and 14(7.0%)cases for benign recurrent vertigo;18(8.1%)cases were central vestibular vertigo,including 11(5.5%)cases for vestibular migraine;8(4.0%)cases were non-vestibular vertigo;and no definite diagnosis was found in 11(5.5%)cases.Conclusions Among hospitalized vertigo patients,nearly 60%are female,and those patients aged 60-69 years account for the largest proportion.Peripheral vertigo,central vertigo and non-vestibular vertigo account for 81%,9%and 4%,respectively.6 percent of patients haven’t received definite diagnosis.BPPV,MD,VN,SHL and BRV are the five common vertigo diseases.
