摘要
目的:观察低分子肝素钠治疗重症肺心病患者的效果。方法:选取100例重症肺心病患者,根据治疗方案不同分为观察组和对照组,每组50例。对照组给予常规治疗,观察组在对照组基础上给予低分子肝素钠治疗,比较两组临床疗效、动脉血气指标水平和血流动力学指标水平、肺功能恢复正常时间和住院时间。结果:观察组治疗总有效率为94.00%,高于对照组的78.00%,差异有统计学意义(P<0.05)。治疗后,两组PaCO2水平均低于治疗前,且观察组低于对照组;两组PaO2水平均高于治疗前,且观察组高于对照组,差异均有统计学意义(P<0.05)。治疗后,两组血细胞比容、血浆黏度、全血低切黏度、全血高切黏度均低于治疗前,且观察组低于对照组,差异均有统计学意义(P<0.05)。观察组肺功能恢复正常时间和住院时间均短于对照组,差异有统计学意义(P<0.05)。结论:在常规治疗基础上采用低分子肝素钠治疗重症肺心病患者可提高治疗总有效率,改善动脉血气指标水平,降低血流动力学指标水平,以及缩短肺功能恢复正常时间和住院时间的效果优于单纯常规治疗效果。
Objective:To observe effects of low-molecular-weight heparin sodium on patients with severe pulmonary heart disease.Methods:100 patients with severe pulmonary heart disease were selected and divided into experimental group and control group,50 cases in each group.The control group was given conventional treatment,while the experimental group was given low-molecular-weight heparin sodium on the basis of that of the control group.The clinical efficacy,the levels of arterial blood gas analysis indexes and hemodynamic index level,the lung function recovery time and the hospital stay were compared between the two groups.Results:The total effective rate t in the experimental group was 94.00%,which was higher than 78.00%in the control group,and the difference was statistically significant(P<0.05).After the treatment,the PaCO2 levels of the two groups were lower than those before the treatment,and that of the experimental group was lower than that of the control group;the PaO2 levels of the two groups were higher than those before the treatment,and that of the experimental group was higher than that of the control group;and the differences were statistically significant(P<0.05).After the treatment,the levels of hematocrit,plasma viscosity,whole blood lowshear viscosity,and whole blood high-shear viscosity of the two groups were lower than those before the treatment;those of the experimental group were lower than those of the control group;and the differences were statistically significant(P<0.05).Further,the lung function recovery time and the hospital stay in the experimental group were shorter than those in the control group,and the differences were statistically significant(P<0.05).Conclusions:On the basis of conventional treatment,the low-molecular-weight heparin in the patients with severe pulmonary heart disease can increase the total treatment effective rate,improve the levels of arterial blood gas analysis indexes,reduce the hemodynamic index level,and shorten the lung function recovery time and the hospital stay
