摘要
目的了解男男性行为者(MSM)安全感缺乏的主观原因,分析其关联因素。方法2018年2-10月运用滚雪球抽样法,对四川省绵阳市MSM进行安全感缺乏主观原因等横断面调查和血清学检测。结果调查问卷1132份,安全感缺乏占83.5%,40.2%因社会歧视、29.8%因健康风险、29.9%因认知偏差等。不同原因的各组之间,HIV、梅毒阳性率无差异(P均>0.05)。安全感缺乏主观原因的关联因素多因素分析,有独立统计学关联的变量:健康风险与社会歧视比:居住地区、社区网络、性生活满意度、接受预防服务、多性伴、高频率肛交;认知偏差等与社会歧视比:年龄、性生活满意度、接受预防服务、寻找性伴途径、多性伴、肛交保护;健康风险与认知偏差等比:年龄、居住地区、性生活满意度、接受预防服务、寻找性伴途径、高频率肛交(P均<0.05)。结论MSM安全感低,其主观原因与社会行为相关,不同原因具不同艾滋病感染风险,应针对性强化行为干预。
Objective To understand the subjective reasons for lack of security among men who have sex with men(MSM)and analyze the related factors.Methods From February to October 2018,the snowball sampling method was used in Mianyang city,Sichuan province to take a cross-sectional survey about the reasons for lack of security with serology testing among MSM.Results Totally 1132 valid questionnaires were collected.83.5%of them felt lack of security,with 40.2%thinking it due to social discrimination,29.8%due to health risks and 30.0%due to cognitive biases.There was no statistical difference in the positive rates of HIV and syphilis among the groups with different reasons for lack of security(P>0.05).The multivariate analysis for the factors of lack of security indicated that there was independent statistical association among the variables such as health risks versus social discrimination:residential area,community network,sexual satisfaction,access to preventive services,multi-sexual partners,and high frequency of anal sex;cognitive biases versus social discrimination:age,sexual satisfaction,receiving preventive services,ways of looking for sexual partners,multi-sexual partners,protection of anal sex;health risk versus cognitive biases:age,residential area,sexual satisfaction,access to preventive services,ways of finding sexual partners,and high frequency of anal sex(P<0.05).Conclusion MSM has a low sense of security.The subjective reasons are related to social behaviors.Therefore,targeted behavior intervention should be reinforced due to different HIV infection risks by different causes.
