全膝关节表面置换术与单间室膝关节置换术治疗老年膝关节单间室骨关节炎的近期临床效果比较 被引量:6

摘要 目的比较全膝关节表面置换术(TKA)与单间室膝关节置换术(UKA)治疗老年膝关节单间室骨关节炎的近期临床效果。方法回顾性分析2018年1月~2020年1月我院老年膝关节单间室骨关节炎患者60例,依据手术方法分为TKA组(n=30)和UKA组(n=30),统计分析两组的视觉模拟评分法(VAS)、膝关节活动度(ROM)、西安大略和麦克马斯特大学骨关节炎指数(WOMAC)、美国特种外科医院(HSS)评分、临床疗效。结果两组术后VAS评分、WOMAC评分均低于术前,差异有统计学意义(P<0.05);ROM、HSS评分均高于术前,差异有统计学意义(P<0.05);手术前后两组VAS评分、ROM、WOMAC评分、HSS评分比较,差异均无统计学意义(P>0.05)。在膝关节功能恢复的优良率方面,TKA组为90.0%(27/30),UKA组为93.3%(28/30),两组比较差异无统计学意义(P>0.05)。结论TKA和UKA治疗老年膝关节单间室骨关节炎的近期临床效果相当。 Objective To compare the short-term clinical effect of total knee arthroplasty(TKA)and unicompartmental knee arthroplasty(UKA)in the treatment of old patients with knee joint unicompartmental osteoarthritis.Methods A total of 60 cases of elderly patients with knee joint unicompartmental osteoarthritis in our hospital from January 2018 to January 2020 were retrospectively analyzed.According to the operation methods,they were divided into TKA group(n=30)and UKA group(n=30).The visual analogue score(VAS),knee mobility(ROM),Western Ontario and McMaster Arthritis Index(WOMAC),American Special Surgery Hospital(HSS)scores and clinical effect of the two groups were statistically analyzed.Results After operation,the VAS score and WOMAC score of the two groups were lower than those before operation,the differences were statistically significant(P<0.05),ROM and HSS scores were higher than those before operation,the differences were statistically significant(P<0.05);before and after operation,the VAS score,ROM,WOMAC score and HSS score of the two groups were not statistically significant(P>0.05).In terms of excellent and good rate of knee joint function recovery,TKA group was 90.0%(45/30),UKA group was 93.3%(46/30),the difference between the two groups was not significant(P>0.05).Conclusion TKA and UKA have the same clinical effect in the treatment of knee joint unicompartmental osteoarthritis in the elderly.
