摘要
目的探讨输尿管软镜鞘在经尿道输尿管硬镜钬激光碎石术处理输尿管大结石中的应用效果。方法回顾性分析2018年11月~2019年10月我院收治的60例输尿管大结石患者的临床资料,根据手术方式的不同将其分为对照组与试验组,每组各30例。对照组患者施以经尿道输尿管镜下钬激光治疗输尿管结石手术,试验组患者施以经尿道输尿管软镜鞘下输尿管镜钬激光碎石治疗输尿管结石手术。观察两组患者的手术时间﹑清石率﹑输尿管损伤发生率、术后并发症总发生率、治疗前后血肌酐与血红蛋白水平等。结果试验组患者的手术时间短于对照组,清石率高于对照组,输尿管损伤发生率及术后并发症总发生率低于对照组,差异均有统计学意义(P<0.05);两组患者治疗前后的血肌酐与血红蛋白水平比较,差异无统计学意义(P>0.05)。结论输尿管软镜鞘在经尿道输尿管硬镜钬激光碎石术处理输尿管大结石中的应用具有显著的临床效果。
Objective To explore the application effect of flexible ureteroscope sheath in transurethral hard-lens ureteral holmium laser lithotripsy for treatment of large ureteral calculi.Methods The clinical data of 60 patients with large ureteral calculi admitted to our hospital from November 2018 to October 2019 were analyzed retrospectively and divided into the control group and the experimental group according to different surgical methods,with 30 cases in each group.The patients in the control group were treated by transurethral holmium laser under ureteroscope,while the patients in the experimental group were treated by flexible ureteroscope sheath in transurethral hard-lens ureteral holmium laser lithotripsy.The operation time,stone clearing rates,the incidence of ureteral injury,the total incidence of postoperative complications,and the levels of serum creatinine and hemoglobin before and after treatment of the two groups were compared.Results The operation time of the experimental group was shorter than that of the control group,the stone clearing rate was higher than that of the control group,the incidence of ureteral injury and the total incidence of postoperative complications were lower than those of the control group,and the differences were statistically significant(P<0.05).There were no statistically significant difference in the level of serum creatinine and hemoglobin between the two groups before and after treatment(P>0.05).Conclusion The application of flexible ureteroscope sheath in transurethral hard-lens ureteral holmium laser lithotripsy for treatment of large ureteral calculi obtains a remarkable clinical effect.
