摘要
目的研究急性脑出血患者血清补体C3、C4水平与脑水肿体积及临床预后的关系。方法选择2016年11月-2017年11月本院收治的100例脑出血患者作为研究对象,所有患者均采用放射性免疫吸附法检测血清补体C3、C4水平,探讨血清补体C3、C4水平与患者脑水肿、临床特点的关系,Logistic回归分析探讨急性脑出血患者预后不良的独立危险因素。结果C3、C4低水平组患者出血体积、出血风险因素高于对应高水平的患者(P<0.05),而出血部位、病因分类无明显差异。C3低水平患者治疗后7 d、14 d脑水肿体积高于对应高水平的患者(P<0.05),C4低水平的患者治疗后7 d、14 d脑水肿体积低于对应高水平的患者(P<0.05),C3、脑水肿体积、C4、出血量是脑出血患者预后不良的独立风险因素(P<0.05)。结论脑出血患者血清中补体C3、C4水平与脑水肿体积、预后密切相关。
Objective To study the relationship between serum complements C3 and C4 levels,brain edema volume and clinical prognosis in patients with acute cerebral hemorrhage.Methods A total of 100 patients with cerebral hemorrhage admitted to our hospital from November 2016 to November 2017 were selected as the research objects.Serum complement C3 and C4 levels were detected by radio immunosorbent assay in all patients.The relationship between serum complement C3,C4 levels,cerebral edema and clinical characteristics was discussed.Logistic regression analysis was used to explore the independent risk factors for poor prognosis of patients with acute cerebral hemorrhage.Results The risk factors of bleeding volume and bleeding risk in patients with low C3 and C4 levels were higher than those of patients with corresponding high levels,but there was no significant difference in bleeding location and etiology classification.The volume of brain edema in patients with low C3 level was significantly higher than that in patients with corresponding high level at 7 d and 14 d after treatment.The volume of brain edema in patients with low C3 level at 7 and 14 days after treatment were significantly lower than that in patients with corresponding high level.C3,brain edema volume,C4 and bleeding volume were independent risk factors for the poor prognosis of patients with cerebral hemorrhage.Conclusion The levels of complement C3 and C4 in serum of patients with cerebral hemorrhage are closely related to the volume and prognosis of cerebral edema.
