摘要
目的探讨SE-EPI和TSE两种T1加权黑血序列在儿童心脏磁共振成像(Magnetic Resonance Imaging,MRI)中的应用价值,比较两种序列对儿童心肌的显示程度。方法纳入来我院行心脏MRI检查的儿童20例,进行黑血成像时采用SE-EPI和TSE两种T1黑血序列扫描并记录诊断结果。由两名医生运用5分制盲法评价图像质量,≥3分者可以诊断,<3分者不能用于诊断。最后统计分析两名医生盲法评分的Kappa值。结果20例儿童中,心肌炎2例,心肌病1例,心律失常4例,先心病2例,代谢性疾病5例,心肌致密化不全1例,心内膜弹力纤维增生症1例,未见异常4例。对于SE-EPI序列,得4分者5例,得3分者8例,得2分者4例,得1分者3例;对于TSE序列,得4分者3例,得3分者12例,得2分者3例,得1分者2例。两名医生盲法评分的Kappa值为0.78。结论SE-EPI序列图像的心肌对比度高于TSE序列,但对受检者心律、呼吸的要求高于TSE序列。心律齐、呼吸平稳的儿童,宜采用SE-EPI序列;心律不齐、呼吸幅度较大的儿童,宜采用TSE序列。
Objective To investigate the application value of SE-EPI and TSE T1-weighted black blood sequences in children’s cardiac magnetic resonance imaging, and compare the display degree of the two sequences in children’s myocardium. Methods A total of 20 children who underwent cardiac MRI examination in our hospital were included. All patients were scanned with SE-EPI and TSE T1 black blood sequences and the diagnosis results were recorded. Two doctors used the 5-point blind method to evaluate the image quality, and those with more than 3 points could be diagnosed, while those with less than 3 points could not be used for diagnosis. Finally, kappa values of two doctor’s,blind scores were statistically analyzed. Results Among the 20 children, there were 2 cases of myocarditis, 1 cases of myocardiosis, 4 cases of arrhythmia, 2 cases of congenital heart disease, 5 cases of metabolic disease, 1 case of incomplete in compact of myocardium, 1 case of hyperplasia in spandex endocardium, 4 cases of normal. For the sequence of SE-EPI, 5 cases with score 4, 8 cases with score 3, 4 cases with score 2, 3 cases with score 1. For the sequence of TSE, 3 cases with score 4, 12 cases with score 3, 3 cases with score 2, 2 cases with score 1. Kappa value between the two observerswas 0.78. Conclusion The myocardial contrast of SE-EPI sequence is higher than that of TSE sequence, but the requirement of heart rhythm and respiration is higher than that of TSE sequence. SE-EPI sequence is suitable for children with arrhythmia and stable breathing, while TSE sequence is suitable for children with arrhythmia and large breathing range.
