摘要
目的探讨青年自发性气胸合并肺大疱治疗中单孔电视辅助胸腔镜手术(VATS)的效果。方法前瞻性选取2017年1月至2020年1月亳州市人民医院新院收治的80例青年自发性气胸合并肺大疱患者作为研究对象,采用随机数字表法分为两组:研究组(n=40)和对照组(n=40)。研究组(选用单孔VATS治疗)和对照组(选用三孔VATS治疗)。比较两组患者术前1 d、术后3 d疼痛评分与手术时间、术中出血量、手术切口长度、住院时间及并发症发生率。结果研究组较对照组手术切口长度[(4.02±0.53)cm vs.(8.61±0.85)cm]、住院时间[(7.41±1.86)d vs.(9.03±2.21)d]更短,疼痛评分[(3.91±0.72)分vs.(6.82±1.07)分]更低,并发症发生率(5.00%vs.22.50%)更少,差异有统计学意义(P<0.05)。两组患者手术时间[(82.18±5.76)min vs.(81.35±4.41)min]、术中出血量[(147.33±19.72)mL vs.(151.18±16.47)mL]比较,差异无统计学意义(P>0.05)。结论青年自发性气胸合并肺大疱治疗中单孔VATS的效果优于三孔VATS,可有效减轻患者术后疼痛程度、手术切口更小、并发症更少,可促进患者快速康复。
Objective To investigate the efficacy of video-assisted thoracic surgery(VATS)in the treatment of spontaneous pneumothorax with bullae in young adults.Methods A total of 80 young patients with spontaneous pneumothorax complicated with bullae admitted to New Hospital of Bozhou People's Hospital from January 2017 to January 2020 were selected as the study subjects.They were randomly divided into two groups using the random number table method,each group 40 patients.The study group was treated with single-well VATS,while the control group was treated with three-well VATS.The pain scores,operative time,intraoperative blood loss,incision length,length of stay and incidence of complications were compared between the two groups 1 d before operation and 3 d after operation.Results Compared with the control group,the study group had shorter incision length[(4.02±0.53)cm vs.(8.61±0.85)cm],shorter hospital stay[(7.41±1.86)d vs.(9.03±2.21)d],lower pain score[(3.91±0.72)points vs.(6.82±1.07)points]and fewer complications(5.00%vs.22.50%),the differences were statistically significant(P<0.05).There were no statistical differences in operative time[(82.18±5.76)min vs.(81.35±4.41)min]and intraoperative blood loss[(147.33±19.72)mL vs.(151.18±16.47)mL]between the two groups(P>0.05).Conclusion The effect of single-hole VATS in the treatment of spontaneous pneumothorax complicated with bullae in youth is better than that of three-hole VATS,which can effectively reduce the postoperative pain of patients with smaller incision and fewer complications,and can promote the rapid recovery of patients.
