摘要
目的评估慢性阻塞性肺病(COPD)患者的运动恐惧程度,并探讨COPD患者发生运动恐惧症的相关因素。方法2019年3-9月收集珠海市人民医院收治的31例GOLDⅡ级和Ⅲ级的COPD患者(COPD组)的病例资料。同时招募31例健康志愿者作为对照组。采集患者的一般人口学和社会学资料。采用疲劳严重程度量表(FSS)评估疲劳程度,根据视觉模拟评分(VAS)(0~10 cm)记录有关慢性疼痛严重程度及其位置,采用改良的英国医学研究委员会呼吸困难量表(mMRC)评估患者呼吸困难水平,共病水平采用Charlson共病指数(CCI)进行评估,坦帕运动恐惧症量表(TSK)进行运动恐惧症评估。结果90.3%的COPD患者发生运动恐惧症(TSK≥38)。COPD组受试者mMRC、CCI、TSK(P<0.001)得分均高于健康对照组。TSK评分与mMRC评分(r=0.676,P<0.001)、CCI指数(r=0.746,P<0.001)、FSS严重程度(r=0.524,P<0.005)显著相关。结论中/重度COPD患者运动恐惧症的发生率较高。运动恐惧症与COPD患者的疼痛、呼吸困难、疲劳严重程度、多系统共病密切相关。临床应常规评估中重度COPD患者是否发生运动恐惧症,以便及早开展有效的心理干预。
Objective To evaluate the degree of motion phobia in patients with chronic obstructive pulmonary disease(COPD),and to investigate the related factors of motion phobia in patients with COPD.Methods From March 2019 to September,31 cases of GOLDⅡandⅢCOPD patients,and 31 healthy volunteers were recruited.The general demographic and sociological data of the patients were collected.The fatigue severity scale(FSS)wasusedto evaluate the degree of fatigue,and the severity and location of chronic pain were recorded according to visual analogue scale(VAS)(0~10 cm).The modified British Medical Research Council dyspnea scale(mMRC)was used to evaluate the dyspnea level.The Charlson comorbidity index(CCI)was used to evaluate the motor phobia.Tampa movement scale was used to evaluate movement phobia(TSK).Results 93%COPD patients had motion phobia(TSK>38).The mMRC,CCI and TSK scores of subjects in the COPD group were higher than those in the healthy control group(P<0.001).TSK score was significantly correlated with mMRC score(r=0.676,P<0.001),CCI index(r=0.746,P<0.001),and FSS severity(r=0.524,P<0.005).Conclusion The incidence of motion phobia is higher in patients with moderate/severe COPD.Motion phobia is closely related to pain,dyspnea,fatigue severity,and multisystem comorbidity in patients with COPD.In order to carry out effective psychological intervention as early as possible,the patients with moderate and severe COPD should be routinely evaluated for the occurrence of motion phobia.
