摘要
目的探讨自制气体反压平台用于经脐单孔腹腔镜子宫次全切除术的安全性和可行性。方法2019年12月~2020年4月,利用自制气体反压平台和常规腹腔镜器械行经脐单孔腹腔镜子宫次全切除术12例。用常规吸引管制成直径6 cm内环、直径7 cm外环,6.5号手套从内环穿过反折于内环上,内环经脐部2 cm切口置入腹腔内,外环紧扣在手套上封闭切口。在4个指套上剪1~2 mm环形小孔,置入输气管、腹腔镜、次操作钳及一个5 mm trocar(主操作钳),手术时各器械及部分指套送入相应指套形成折叠,气体反压防止漏气。结果均成功完成经脐单孔腹腔镜子宫次全切除术,无漏气。标本装入标本袋内取出,标本袋完整。术后4~5 d痊愈出院。无并发症发生。结论自制简易平台制作简单,解决漏气问题,不增加耗材,符合卫生经济学,对有经验的术者开展经脐单孔腹腔镜手术是安全可行的。
Objective To investigate the safety and feasibility of self-made gas reverse pressure platform for transumbilical single-port laparoscopic subtotal hysterectomy.Methods From December 2019 to April 2020,12 cases of transumbilical single-port laparoscopic subtotal hysterectomy were performed with self-made gas reverse pressure platform and conventional laparoscopic instruments.A conventional suction tube was used to form an inner ring with a diameter of about 6 cm and an outer ring with a diameter of 7 cm.A silk thread was attached to the inner ring.A 6.5#glove was passed through the inner ring,and the glove was folded over the inner ring.The outer ring was fastened to the glove to close the cut.Four 1-2 mm annular holes were cut on the four finger sets and the air duct,laparoscope,secondary operating forceps and a 5 mm trocar(main operating forceps)were placed into.During the operation,each instrument and part of the finger sleeve were fed into the corresponding finger sleeve to form folding gas reverse pressure to prevent leakage.Results Subtotal transumbilical laparoscopic hysterectomy was successfully performed in all the patients without air leakage.The specimen was taken out in a complete specimen bag.All the patients recovered and were discharged from the hospital within 4-5 days after the operation.No complications occurred.Conclusions The self-made simple platform is easy to make.It can solve the problem of air leakage,does not add consumables,conforms to health economics,and is safe and feasible for experienced operators to carry out transumbilical single-hole laparoscopic surgery.
