摘要
目的:探讨高流量氧疗联合盐酸纳洛酮在呼吸衰竭患者中的临床效果及对呼吸力学的影响。方法:选择2018年4月-2020年1月呼吸衰竭患者94例作为对象,按随机数字表法分为两组,各47例。对照组采用高流量氧疗治疗,观察组在对照组基础上联合盐酸纳洛酮治疗,两组均完成7 d治疗,治疗后完成3个月随访,比较两组痰液黏稠度、呼吸动力学、并发症及死亡率。结果:观察组治疗后7 d痰液黏稠度Ⅱ级病例数多于对照组(P<0.05);痰液黏稠度Ⅲ级、Ⅳ级病例数少于对照组(P<0.05);观察组治疗后7 d气道峰值低于对照组,呼吸频率、潮气量高于对照组,差异均有统计学意义(P<0.05);两组治疗过程中胃肠胀气、吸入性肺炎、感染发生率及治疗后1、2个月死亡率比较差异无统计学意义(P>0.05);观察组治疗后3个月死亡率低于对照组(P<0.05)。结论:高流量氧疗联合盐酸纳洛酮用于呼吸衰竭患者中能降低痰液黏稠度分级,改善患者呼吸动力学水平,能降低临床死亡率,未增加并发症发生率,值得推广应用。
Objective:To explore the clinical effect of high-flow oxygen therapy combined with Naloxone Hydrochloride in patients with respiratory failure and its influence on respiratory mechanics.Method:A total of 94 patients with respiratory failure from April 2018 to January 2020 were selected as subjects.They were divided into two groups by random number table method,47 cases in each group.The control group was treated with high-flow oxygen therapy.The observation group was combined with Naloxone Hydrochloride treatment on the basis of the control group.Both groups completed 7 days of treatment,and completed 3 months of follow-up after treatment.The sputum viscosity grade,respiratory dynamics,complications and mortality were compared.Result:Severn days after treatment,the observation group had more cases of sputum viscosity grade Ⅱ than the control group (P<0.05).The number of sputum viscosity grades Ⅲ and Ⅳ of the observation group was less than that of the control group (P<0.05).The airway peak value in the observation group was lower than that in the control group after 7 days,and the respiratory frequency and tidal volume were higher than those of the control group,the differences were statistically significant (P<0.05).The incidence of flatulence,aspiration pneumonia,and infection during the treatment of the two groups,there was no statistically significant difference in the mortality of 1 and 2 months after treatment (P>0.05).The mortality of the observation group was lower than that of the control group at 3 months after treatment (P<0.05).Conclusion:High-flow oxygen therapy combined with Naloxone Hydrochloride in patients with respiratory failure can reduce the sputum viscosity grade,improve the respiratory dynamics of patients,can reduce clinical mortality,and does not increase the incidence of complications.It is worthy of popularization and application.
