摘要
目的探讨血清降钙素原(PCT)与C反应蛋白(CRP)在新生儿败血症早期诊断中的应用价值。方法采用回顾性研究方法,选取73例临床诊断为新生儿败血症患儿作为败血症组(其中日龄<3 d者43例,3~30 d者30例),另选同期住院的76例无感染新生儿作为对照组(其中日龄<3 d者35例,3~30 d者41例)。分别检测两组新生儿血清中PCT、CRP水平,并比较PCT、CRP单独检测及联合检测对早期新生儿败血症的诊断结果。结果败血症组新生儿PCT、CRP水平均明显高于对照组(P<0.05)。在日龄3~30 d及日龄<3 d的败血症患儿中,与PCT、CRP单独检测比较,PCT与CRP联合检测诊断早期新生儿败血症的特异度、阳性预测值及约登指数均较高(P<0.05)。结论PCT与CRP联合检测有助于提高早期新生儿败血症诊断的特异度和准确度。
Objective To investigate the application value of serum procalcitonin(PCT)and C-reactive protein(CRP)in early diagnosis of neonatal sepsis.Methods Using retrospective research methods,73 children who were clinically diagnosed with neonatal sepsis were selected as the sepsis group(43 cases were less than 3 days old and 30 cases were 3 to 30 days old),in addition,76 non-infected newborns who were hospitalized during the same period were selected as the control group(35 cases were less than 3 days old and 41 cases were 3 to 30 days old).The levels of serum PCT and CRP in the two groups were respectively detected.Diagnostic values of PCT,CRP and the combination of PCT and CRP for early neonatal sepsis were evaluated.Results The levels of both PCT and CRP in the sepsis group were significantly higher than that in the control group(P<0.05).The specificity,positive predictive value and Youden index of the combined detection of PCT and CRP for children with neonatal sepsis aged 3 to 30 days and age less than 3 days were higher than that of PCT or CRP testing alone in the diagnosis of early neonatal sepsis.Conclusion The combination test of PCT and CRP could help improve specificity and accuracy in the early diagnosis of neonatal sepsis.
