摘要
[目的]分析经胸腔镜肺叶(段)切除术肺部恶性肿瘤患者住院总费用、一次性材料费用的影响因素,为合理控制患者住院费用提供参考依据。[方法]提取不同医改政策下经胸腔镜肺叶(段)切除术切除肺部恶性肿瘤患者的住院费用和病案首页信息,对其进行描述性统计及逐步线性回归分析。[结果]回归分析显示,住院天数、新技术使用数量、医改政策、麻醉次数的t值分别为17.22、17.69、3.50、3.11,均为P<0.01;其他诊断数量的t值为2.77,P<0.05。[结论]住院天数、采用新技术的数量、取消耗材加成政策是影响经胸腔镜下肺叶切除术患者住院费用的主要因素。建议医保部门加强对新医疗技术的评估,逐步将疗效显著、费用负担可承受的新技术纳入医保报销目录。
Objective To analyze the influencing factors of hospitalization total expenses and the expenses of one-time materials for patients with lung cancer who underwent thoracoscopic lobectomy,so as to provide reference basis for rationally controlling the expenses of hospitalization.Methods It extracted inpatient expenses and medical records home page of patients with lung cancer who underwent thoracoscopic lobectomy under different medical reform policies,descriptive statistics and stepwise linear regression analysis were carried out to analyze.Results The regression analysis showed that t value of hospitalization days,number of new technique usage,medical reform and times of anesthesia was 17.22,17.69,3.50 and 3.11 respectively,all were P<0.01,other of number of diagnosis and treatment was 2.77,P<0.05.Conclusions Length of stay,number of new technology,policy of canceling consumable addition are main factors which influence hospitalization expenses of patients with lung cancer who underwent thoracoscopic lobectomy.It suggests that medical insurance department should strengthen the evaluation of new technology,bring new technology with effective treatment and affordable expense into medical insurance reimbursement list.
