摘要
目的探究高频彩超对乳腺叶状肿瘤和纤维腺瘤的鉴别诊断效果。方法择取2018年12月—2020年1月期间该院收治的42例乳腺叶状肿瘤患者和45例纤维腺瘤患者参与该次研究,所有患者均行病理检查确诊,为两组患者分别实施高频彩超检查,对两组肿瘤直径大小、超声特征及血流信号情况进行比较。结果在超声特征乳腺叶状肿瘤肿瘤直径(35.48±4.62)mm大于纤维腺瘤(20.12±2.25)mm,两组患者差异有统计学意义(t=19.496,P<0.05);乳腺叶状肿瘤的钙化率(71.43%)、无回声区(78.57%)、形态不规则度71.43%、后方回声增强度(73.81%)、内部回声不均(76.19%)多于纤维腺瘤的33.33%、44.44%、37.78%、35.56%、46.67%,两组患者差异有统计学意义(χ2=12.626、10.627、9.904、12.799、7.954,P<0.05);乳腺叶状肿瘤的血流信号分级0级(11.90%)、Ⅰ级(21.43%)、Ⅱ级(40.48%)、Ⅲ级(26.19%),纤维腺瘤的血流信号分级0级(44.44%)、Ⅰ级(37.78%)、Ⅱ级(13.33%)、Ⅲ级(4.44%),两组患者差异有统计学意义(Z=21.406,P<0.05)。结论通过高频彩超技术鉴别诊断乳腺叶状肿瘤和纤维腺瘤效果确切,可以为患者的临床治疗提供科学依据,有利于针对性开展治疗。
Objective To explore the differential diagnosis effect of high-frequency color Doppler ultrasound on phyllodes tumors and fibroadenomas of the breast.Methods From December 2018 to January 2020,42 patients with phyllodes breast tumors and 45 patients with fibroadenoma admitted to our hospital were selected to participate in this study.All patients were diagnosed by pathological examination and implemented for two groups of patients.High-frequency color Doppler ultrasound examination to compare the tumor size,ultrasound characteristics and blood flow signals of the two groups.Results The ultrasonographic characteristics of breast phyllodes tumor diameter(35.48±4.62)mm were greater than fibroadenoma(20.12±2.25)mm.The difference between the two groups was statistically significant(t=19.496,P<0.05);Calcification rate(71.43%),non-echo area(78.57%),morphological irregularity 71.43%,rear echo enhancement(73.81%),internal echo unevenness(76.19%)were more than that of fibroadenomas 33.33%,44.44%,37.78%,35.56%,46.67%,the difference between the two groups was statistically significant(χ2=12.626,10.627,9.904,12.799,7.954,P<0.05);blood flow signal of breast phyllodes tumor grade 0(11.90%),grade I(21.43%),grade II(40.48%),grade III(26.19%),fibroadenoma blood flow signal grade 0(44.44%),grade I(37.78%),grade II(13.33%),gradeⅢ(4.44%),the difference between the two groups of patients was statistically significant(Z=21.406,P<0.05).Conclusion The differential diagnosis of breast phyllodes tumor and fibroadenoma by high-frequency color Doppler ultrasound technology is definite and can provide a scientific basis for the clinical treatment of patients,which is conducive to targeted treatment.
