摘要
目的探讨微创点状剥脱术治疗大隐静脉曲张的效果及安全性。方法选取2017年5月—2019年10月该院收治的120例大隐静脉曲张患者,随机分为两组,对照组应用传统剥脱术,研究组使用微创点状剥脱术,对比两组患者干预效果。结果研究组患者手术时间、住院时间以及出血量均明显低于对照组[(29.4±5.3)min、(5.6±2.1)d、(38.7±4.2)mL vs(56.8±7.5)min、(13.4±3.9)d、(94.2±6.8)mL],差异有统计学意义(t=23.111、13.640、53.788,P=0.000);研究组并发症发生率低于对照组(10.0%vs 31.7%)(χ2=8.539,P=0.003<0.05);治疗前两组患者VCSS、CIVIQ评分无差异无统计学意义(P>0.05),治疗4周后研究组患者的VCSS评分和CIVIQ评分均优于对照组[(1.5±0.4)分、(86.5±7.1)分vs(3.2±1.1)分、(73.6±6.8)分],差异有统计学意义(t=11.250、10.164,P=0.000)。结论微创点状剥脱术治疗大隐静脉曲张患者效果显著,创伤小、术后复发率和并发症发生率降低,缩短住院时间。
Objective To explore the effect and safety of minimally invasive point stripping in the treatment of great saphenous varices.Methods From May 2017 to October 2019,120 patients with great saphenous varicose veins were randomly divided into two groups,the control group was treated with traditional exfoliation,and the research group was treated with minimally invasive spotted exfoliation.The intervention effect of the two groups were compared.Results The operation time,hospital stay and bleeding volume of the study group were lower than that of the control group[(29.4±5.3)min,(5.6±2.1)d,(38.7±4.2)mL vs(56.8±7.5)min,(13.4±3.9)d,(94.2±6.8)mL],the differences were statistically significant(t=23.111,13.640,53.788,P=0.000);the incidence of complications in the study group was lower than that in the control group(10.0%vs 31.7%),the difference was statistically significant(χ2=8.539,P=0.003<0.05);there was no significant difference in VCSS and CIVIQ scores between the two groups before treatment(P>0.05).After 4 weeks of treatment,the VCSS score and CIVIQ score of the study group were better than the control group[(1.5±0.4)points,(86.5±7.1)points vs(3.2±1.1)points,(73.6±6.8)points]),the difference was statistically significant(t=11.250,10.164,P=0.000).Conclusion Minimally invasive spotted ablation is effective in treating patients with great saphenous varicose veins.It has less trauma,lower postoperative recurrence rate and complication rate,and shortens the length of hospital stay.
