摘要
目的:探讨术前应用益生菌对结直肠癌患者围手术期营养及肠黏膜屏障功能改善作用。方法:选择2017年2月-2019年12月德州市人民医院收治的80例结直肠癌手术患者作为研究对象,按照随机数表法分为观察组与对照组,每组各40例。对照组患者术前给予常规营养支持,观察组患者在对照组基础上术前加用双歧杆菌三联活菌治疗。对比两组患者手术前后营养状况、肠黏膜屏障功能与免疫功能水平变化。结果:术后3 d,两组患者Hb、Alb水平较术前1 d高,且观察组患者更高,差异有统计学意义(P<0.05);两组患者DAO、内毒素水平较术前1 d高,但观察组患者比对照组患者低,差异有统计学意义(P<0.05);两组患者CRP、TNF-α水平较术前1 d高,但观察组患者较对照组患者低,差异有统计学意义(P<0.05)。结论:术前应用益生菌于结肠癌患者中可有效改善患者围手术期营养与肠黏膜屏障功能,且能够减轻手术引起的炎症反应,值得临床应用推广。
Objective: To investigate the effect of preoperative probiotics(bifidobacterium trifecta) on perioperative nutrition and intestinal mucosal barrier function in colorectal cancer patients.Methods: 80 cases of colorectal cancer treated with surgical treatment in our hospital from February 2017 to December 2019 were selected. According to the random number table method, they were divided into the observation group and the control group, with 40 cases in each group. Patients in the control group were given routine nutritional support before surgery, and patients in the observation group were treated with bifidobacteria triple live bacteria on the basis of patients in the control group. The changes of nutritional status, intestinal mucosal barrier function and immune function before and after operation were compared between the two groups. Results: 3 days after the operation, Hb and Alb levels in the two groups were higher than those 1 day before the operation, and those in the observation group were higher, the difference was statistically significant(P<0.05). The levels of DAO and endotoxin in the two groups were higher than that 1 d before surgery, but the level in the observation group was lower than that in the control group, and the difference was statistically significant(P<0.05). CRP and TNF-levels in the two groups were higher than those 1 d before surgery, but the level in the observation group was lower than that in the control group, and the difference was statistically significant(P<0.05). Conclusion: Preoperatively applying probiotics to colon cancer patients can effectively improve perioperative nutrition and intestinal mucosal barrier function, and reduce the inflammatory response caused by surgery.
