摘要
目的研究乙肝肝硬化患者体成分分析在评估患者预后及生存率中的应用价值。方法选取2018年3月至2019年5月我院收治的180例乙肝肝硬化患者,将在观察期内死亡的患者(死亡原因均与肝病有关)归为死亡组(n=52),未死亡的患者归为生存组(n=128)。对两组患者的身体质量指数(BMI)、体细胞量(BCM)、细胞外液(ECW)、细胞内液(ICW)、身体总水(TBW)、总体脂肪(TBF)以及各指标预测1年预后生存率的准确性进行比较。结果死亡组的BMI显著低于生存组,差异具有统计学意义(P<0.05)。死亡组的BCM和ICW水平显著低于生存组,ECW和TBW水平显著高于生存组,差异具有统计学意义(P<0.05)。死亡组的TBF显著低于生存组,差异具有统计学意义(P<0.05)。体成分联合指标评估患者1年预后生存率的准确性显著高于体成分各独立指标,差异具有统计学意义(P<0.05)。结论乙肝肝硬化患者体成分分析能够正确评估患者预后及生存率,从而为临床医生治疗乙肝肝硬化提供营养治疗的新思路,具有较高的临床应用价值。
Objective To study the application value of body composition analysis in evaluating the prognosis and survival rate of patients with hepatitis B cirrhosis.Methods One hundred and eighty patients with hepatitis B cirrhosis admitted in our hospital from March 2018 to May 2019 were selected.The patients who died during the observation period(the causes of death were all related to liver disease)were divided into death group(n=52),and the patients who did not die were classified into survival group(n=128).The body mass index(BMI),body cell mass(BCM),extracellular water(ECW),intracellular water(ICW),total body water(TBW),total body fat(TBF)and the accuracy of each index in predicting 1-year survival rate were compared between the two groups.Results The BMI of the death group was significantly lower than that of the survival group,and the difference was statistically significant(P<0.05).The levels of BCM and ICW in the death group were significantly lower than those in the survival group,while the levels of ECW and TBW in the death group were significantly higher than those in the survival group,and the differences were statistically significant(P<0.05).The TBF in the death group was significantly lower than that in the survival group,and the difference was statistically significant(P<0.05).The accuracy of 1-year survival rate assessed by the body component combination index was significantly higher than that of each independent index of body composition,and the difference was statistically significant(P<0.05).Conclusion Body composition analysis of patients with hepatitis B cirrhosis can correctly evaluate the prognosis and survival rate,so as to provide clinicians with a new idea of nutritional treatment for hepatitis B cirrhosis,which has high clinical application value.
