摘要
目的观察三黄汤保留灌肠治疗肝性脑病的疗效。方法使用随机平行对照方法,将辽宁中医药大学附属医院消化内科收治的46例住院患者随机分为2组,治疗组24例,对照组22例,两组在支链氨基酸及盐酸精氨酸治疗基础之上,对照组予乳果糖60~80 ml加等量生理盐水稀释后,日1次保留灌肠。治疗组予三黄汤(黄连20 g、黄芩20 g、大黄20 g)150 ml,日1次保留灌肠,两组疗程7 d。依据West-Haven分级标准记录患者精神状态恢复时间,记录治疗前后血氨水平。结果治疗组精神状态改善时间为(26.4±4.5)h,对照组为(36.6±5.8)h,治疗组改善时间显著短于对照组(P<0.05);治疗组精神状态改善20例,有效率83.3%,对照组精神状态改善15例,有效率68.2%,治疗组精神状态疗效显著优于对照组(P<0.05);治疗后,治疗组血氨水平为(48.4±22.2)μmol/L,对照组血氨水平为(69.6±20.8)μmol/L,治疗组血氨水平改善显著优于对照组(P<0.05)。结论三黄汤灌肠治疗能够有效改善肝性脑病患者精神状态,降低患者血氨水平,提高肝性脑病的总体疗效。
Objective To investigate the efficacy on hepatic encephalopathy with retention enema of Sanhuang Decoction.Methods Totally 46 cases with hepatic encephalopathy,treated in the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine,were divided into test group(24 cases)and control group(22 cases)by random parallel control method.All the 46 cases were treated with BCAA and arginine hydrochloride;retention-enema with Sanhuang Decoction was adopted in test group once a day and lactulose was adopted in control group once a day.Before and after the treatment,the clinical symptoms including consciousness and the level of NH in serum was detected.Results The consciousness recovered in(26.4±4.5)h in test group,which was significantly less than(36.6±5.8)h in control group(P<0.05).Twenty cases recovered or were improved,with an improving rate 83.3%in test group,which was significantly higher than that in control group(n=15,68.2%)(P<0.05).The level of ammonia in test group after retention enema was(48.4±22.2)μmol/L,which was significantly lower than(69.6±20.8)μmol/L in control group(P<0.05).Conclusion It has been proved that retention enema With Sanhuang Decoction has a significant clinic effect in hepatic encephalopathy.
