摘要
目的经会阴盆底三维超声及尿失禁生活质量问卷评分评估女性压力性尿失禁(SUI)患者生物反馈电刺激治疗的盆底结构改变及效果分析。方法选择SUI患者符合盆底生物反馈电刺激治疗的患者40例作为研究对象,进行盆底肌电生物治疗,并于治疗前及治疗后,分别进行尿垫试验、盆底肌肌力测定、经会阴盆底三维超声检查及生活质量问卷调查,其中经会阴二维及三维超声于安静状态及最大Valsalva动作时分别测量患者的盆膈裂孔面积、耻骨直肠肌厚度、膀胱颈至耻骨联合下缘的距离、膀胱后角、膀胱颈旋转角度等,并进行治疗前后比较。结果经盆底康复治疗后,1h尿垫试验漏尿量明显减少(P<0.01),盆底肌肌力及收缩压均显著提升(P<0.05);SUI患者张力期膀胱颈(BN)位置(-2.23±2.34)mm较治疗前(-5.72±2.11)mm上移(P<0.05),治疗后膀胱颈移动度(BND)[(25.11±5.44)mm]较治疗前[(28.97±7.11)mm]减少(P<0.05),盆膈裂孔面积和尿道旋转角度均较治疗前缩小,治疗后尿失禁生活质量问卷(I-QOL)评分[(51.68±14.17)分]高于治疗前[(28.98±11.12)分](P<0.05)。结论经会阴盆底超声联合生活质量问卷评分能够更好地评估女性SUI患者生物反馈电刺激疗法的效果及盆底结构改变,给临床治疗提供可靠依据。
Objective To evaluate of the efficacy of biofeedback electrical stimulation on female stress urinary incontinence(SUI)by transperineal pelvic floor three-dimensional(3 D)ultrasound and incontinence quality of life questionnaire(IQOL).Methods A total of 40 SUI patients undergoing pelvic floor biofeedback electrical stimulation were enrolled.Before and after treatment,urine pad test,pelvic floor muscle strength measurement,transperineal pelvic floor 3 D ultrasound,and IQOL were conducted.At ultrasound resting state and the maximum Valsalva action,phrenic fissure area,puborectalis thickness,distance from bladder neck(BN)to the lower edge of pubic symphysis,bladder neck descent(BND),and urethral rotation angle(UR)were measured.The data collected before and after treatment were compared.Results After treatment,urine leakage in the 1-hour urine pad test was significantly reduced(P<0.01);pelvic floor muscle strength and systolic blood pressure were significantly increased(P<0.05);the position of BN in tension phase was higher[(-2.23±2.34)mmvs.(-5.72±2.11)mm,P<0.05];BND was decreased[(25.11±5.44)vs.(28.97±7.11)mm,P<0.05];area of phrenic fissure and urethral rotation angle were smaller;I-QOL score was higher[(51.68±14.17)vs.(28.98±11.12),P<0.05].Conclusion Pelvic floor ultrasound combined with I-QOL can effectively evaluate the efficacy of biofeedback therapy for women with SUI and provide a reliable basis for clinical treatment.
