摘要
目的探讨游离桡动脉掌浅支皮瓣在手部烧伤患者中的应用效果.方法选择2017年12月至2019年1月手部烧伤患者78例,随机分为对照组和观察组,各39例.对照组给予股前外侧皮瓣游离移植,观察组采用游离桡动脉掌浅支皮瓣移植,15d治疗后对患者效果进行评估,随访3个月,比较两组移植皮瓣成活率、视觉模拟疼痛(VAS)评分、外观与活动功能.结果观察组皮瓣成活率高于对照组(P<0.05);观察组术后感染、手部挛缩及瘢痕形成发生率低于对照组(P<0.05);观察组术后3d、5d、15d?VAS评分低于对照组(P<0.05);两组手部创面愈后外观、功能均得到明显改善,观察组治疗后3个月优良率94.77%高于对照组71.79%(P<0.05).结论游离桡动脉掌浅支皮瓣用于手部烧伤患者中能提高皮瓣成功率,降低术后并发症发生率,有助于减轻患者疼痛,可获得良好的外观与活动功能,值得推广应用.
Objeetive To explore the application ffct of free radial artery superficial palmar branch flap in patients with hand burs and its fect on aesthetics.Methods Seventy--eight patients with hand burns from December 2017 to January 2019 were selected as subjects and randomly divided into a control group(n=39 cases)and an observation group(n=39 cases).The control group received free anterior lateral femoral flap transplantation.The observation group received free radial artery superficial palmar branch flap transplantation.Afer 15 days of treatment,the efect of the patients was evaluated.The patients were followed up for 3 months.The survival rates,Visual analogue pain(VAS)score,aesthetics and motor function were compared between the two groups.Results The survival rate of skin flaps in the observation group was higher than that in the control group(P<0.05);the incidence of postoperative infection,hand contracture and scar formation in the observation group was lower than that in the control group(P<0.05).The 15-day VAS score was lower than the control group(P<0.05);the appearance and function of the hand wounds in both groups were significantly improved,and the excellenet and good rate was 94.77%at 3 months afier treatment in the observation group,which was higher than the control group at 71.79%(P<0.05),Conclusion The application of the free nadial superficial palmar branch flap in patients with hand bums can improve the success rate of the flap,reduce the incidence of postoperative complications,help reduce the patient's pain,and obtain good aestherics and mobility.It is worthwhile Promotion and application.
