摘要
目的:分析社区医院全-专联合模式下家庭医生对难治性高血压的干预实践效果。方法:筛选2017年在册管理的35~75岁高血压患者212例,随机分为两组。对照组采用常规干预模式;观察组采用社区医院全-专联合模式下家庭医生干预模式。比较两组干预效果。结果:观察组干预两年后血压水平及不良事件发生率均低于对照组,差异有统计学意义(P<0.05)。观察组服药依从率高于对照组,差异有统计学意义(P<0.05)。观察组血糖、血脂、血压及体重控制良好率均优于对照组,且不良生活习惯改正率优于对照组,差异有统计学意义(P<0.05)。结论:社区医院全-专联合模式下家庭医生干预模式,可提高高血压患者服药依从性,有效控制血糖、血脂、血压水平及体重指数,吸烟、酗酒等情况明显降低。
Objective:To analyze theinterventionpractical effect of family doctor's on refractory hypertension in community hospitals under the whole-specialized combinedmode.Methods:212 hypertension patients aged 35~75 years who were registered in 2017 were screened and they were randomly divided into the two groups.Conventional intervention mode was used in the control group.The observation group adopted the intervention offamily doctor'sin community hospitalsunderthe whole-specialized combined mode.The intervention effects between the two groups were compared.Results:After two years of intervention,the blood pressure level and the incidence of adverse events in the observation group were lower than the control group,withthe differences were statistically significant(P<0.05).The compliance rate of drug administration in the observation group was higher than the control group,withthe difference was statistically significant(P<0.05).The rate of blood glucose,bloodlipid,blood pressure and good weight control in the observation group was better than the control group,and the rate of correcting bad living habits was also better than the control group,withthe differences were statistically significant(P<0.05).Conclusion:Theintervention of family doctors in community hospitals under the whole-specialized combined modecan improve the medication compliance of patients with hypertension,effectively control blood glucose,bloodlipid,blood pressure level and weightindex,which can significantly reduce the condition of smoking and alcoholism.
