摘要
目的:探讨中能量聚焦式体外冲击波治疗肩袖钙化性肌腱炎的临床疗效。方法:将肩袖钙化性肌腱炎患者56例随机分为观察组和对照组各28例。对照组给予常规药物及康复治疗,观察组在对照组的基础上另行中能量(0.12~0.25mJ/mm2)聚焦式体外冲击波治疗,每周1次,共治疗4次。分别于治疗前、治疗2周、治疗4周及治疗结束后8周随访进行疼痛视觉模拟评分(VAS)、肩关节功能评分(UCLA)评定;于治疗4周及治疗结束后8周随访行钙化灶消失有效率比较。结果:治疗2周时,2组患者VAS评分均较治疗前明显下降(均P<0.05),ULCA评分均较治疗前明显提高(均P<0.05),但VAS及ULCA评分2组间比较差异无统计学意义;治疗4周后,2组患者VAS评分均较治疗前及治疗2周时明显下降(均P<0.05),ULCA评分均较治疗前及治疗2周时明显提高(均P<0.05),且观察组VAS评分更低于对照组(P<0.05),观察组ULCA评分更高于对照组(P<0.05);治疗结束后8周随访,2组患者VAS评分均较治疗前、治疗2周及治疗4周时明显下降(均P<0.05),ULCA评分均较治疗前、治疗2周及治疗4周时明显提高(均P<0.05),且观察组VAS评分更低于对照组(P<0.05),观察组ULCA评分更高于对照组(P<0.05)。治疗4周时,观察组肩袖相关钙化灶吸收情况较对照组改善更显著(P<0.05);治疗8周随访时发现,2组患者有效率较治疗4周时均有不同程度的提高(均P<0.05),且观察组有效率更高于对照组(P<0.05)。结论:中能量聚焦式体外冲击波治疗可有效减轻肩袖钙化性肌腱炎患者疼痛、改善肩关节活动功能、促进钙化灶的消失。
Objective:To evaluate the efficacy of medium energy focused extracorporeal shock wave in the treatment of rotator cuff calcific tendinitis.Method:Fifty-six patients with rotator cuff calcific tendinitis were randomly divided into treatment group and control group,28 patients in each group.The control group and treatment group were treated with conventional drug therapy and rehabilitation training.Additionally,the treatment group was given medium energy(0.12-0.25 mJ/mm2)focused extracorporeal shock wave therapy,once a week for 4 weeks.The VAS and UCLA scores of patients were respectively assessed before treatment,2 weeks and 4 weeks of treatment,and at 8 th week after treatment.The disappearance rate of calcified foci was respectively assessed after 4 weeks of treatment,and at 8 th week after treatment.Result:After 2 weeks of treatment:As compared with those before treatment,the VAS scores were significantly reduced,and UCLA scores were significantly increased in two groups(both P<0.05),but there was no statistically significant difference between the two groups.After 4 weeks of treatment,as compared with those before treatment and after 2 weeks of treatment,VAS scores were significantly reduced,and UCLA scores significantly increased in two groups(all P<0.05),and the VAS scores were significantly higher,and the ULCA scores were significantly lower in the treatment group than in the control group(P<0.05).The disappearance rate of calcified foci was more obvious in the treatment group than in the control group(P<0.05).At 8 th week after treatment,as compared with those before treatment,after 2 weeks of treatment and after4 weeks of treatment,VAS scores were significantly reduced,and UCLA scores significantly increased in two groups(P<0.05),and the VAS scores were significantly higher,and the ULCA scores were significantly lower in the treatment group than in the control group(P<0.05).There was significant difference in the disappearance rate of calcified foci between two groups(P<0.05).Conclusion:Medium energy focu
