摘要
目的:观察腹腔镜联合胆道镜胆总管探查一期缝合术(LCBDE+PDC)治疗胆总管结石患者的效果。方法:选取86例胆总管结石患者为研究对象,依据随机数字表法分为观察组和对照组各43例。两组均实施LCBDE,对照组在此基础上采取T管引流术(TTD),观察组采取PDC,比较两组手术相关指标水平、术后相关指标水平和并发症发生率。结果:观察组胃肠道功能恢复时间和术后住院时间均短于对照组,差异有统计学意义(P<0.05);两组术中出血量、手术时间、引流管拔除时间和并发症发生率比较,差异均无统计学意义(P>0.05)。结论:LCBDE联合PDC治疗胆总管结石患者可缩短胃肠道功能恢复时间和术后住院时间的效果优于LCBDE联合TTD治疗效果。
Objective:To observe effects of laparoscopy combined with choledochoscopic common bile duct exploration and primary duct closure(LCBDE+PDC)in treatment of patients with choledocholithiasis.Methods:86 patients with choledocholithiasis,who were treated in the Second People’s Hospital Gynecology and Obstetrics Hospital of Fuxin City from August 2017 to August 2019,were selected as the research objects.According to the random number table method,they were divided into observation group and control group,43 cases in each group.Both groups were treated with laparoscopic cholecystectomy+choledochoscopic common bile duct exploration(LCBDE).On this basis,the control group adopted T-tube drainage(TTD),while the observation group was given PDC.The levels of surgery-related indexes and postoperative related indexes and the incidence of complications were compared between the two groups.Results:The gastrointestinal function recovery time and postoperative hospital stay in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).There were no significant differences in the intraoperative blood loss,operation time,drainage tube removal time,and complication rate between the two groups(P>0.05).Conclusions:LCBDE+PDC in the treatment of choledocholithiasis can shorten the recovery time of gastrointestinal function and the postoperative hospital stay.Moreover,it is superior to LCBDE+TTD.
