摘要
目的探讨肾透明细胞肉瘤(clear cell sarcoma of the kidney,CCSK)的CT表现。方法回顾性分析6例经病理证实的CCSK患者的临床资料及CT平扫和增强图像,总结CT表现。结果4例CCSK位于左肾,2例位于右肾,瘤体最大直径为5.0~15.7 cm,肿瘤表现为较大的边缘清楚软组织肿块,来源于肾髓质,肾皮质受压变薄但无侵犯,肿块密度不均,易坏死。增强扫描肿瘤血供丰富,呈轻-中度强化,淋巴结及骨转移多见,3例患者肿瘤边缘可见小囊状影。结论CCSK的CT表现并无明确的特征性,肿瘤边缘小囊状影可能具有一定的鉴别价值。
Objective To study the CT manifestations of clear cell sarcoma of the kidney(CCSK).Methods Clinical data and CT plain and enhanced images of 6 patients with CCSK confirmed by pathology were analyzed retrospectively.The CT manifestations were summarized.Results The CCSK was located in the left kidney in 4 cases,and in the right kidney in 2 cases.The maximum tumor diameter was 5.0-15.7 cm.The tumor was derived from the renal medulla and showed a large soft tissue mass with clear margin.The renal cortex thinned under compression but was not invaded.All masses had heterogeneous density and appeared to be necrotic.Enhance CT displayed abundant blood supply with a mild to moderate enhancement pattern.Lymph node and bone metastases were commonly seen.Small cystic shadow was observed at the edge of the tumor in 3 cases.Conclusion The CT manifestations of CCSK have no definite characteristics.Peritumoral cysts may have some value in the identification of CCSK.
