基于光流法的颅内外血管旁路移植术中吲哚氰绿血管造影血流半定量分析 被引量:1

摘要 目的探讨Horn-Schunck(HS)光流法结合吲哚氰绿术中造影在脑血流重建术中的应用价值。方法回顾性连续纳入复旦大学附属华山医院神经外科2018年12月至2019年6月烟雾病患者20例,均行颞浅动脉-大脑中动脉旁路移植术,且使用内置Flow800软件的手术显微镜。用HS光流法分析手术前后吲哚氰绿造影视频,通过计算受体血管每个像素点的光流矢量,半定量化受体血管的血流方向。20例患者术前及术后共40个视频,以Flow800软件的判读结果为参考,若HS光流法与Flow800软件对术前视频中血流方向(正向、反向)、术后视频血流方向(正向、反向、中间向两侧血流)判读一致,则认为正确。对每例患者经手术后是否改变血流方向进行判断,若HS光流法与Flow800软件均显示改变了血流方向,则认为一致。将HS光流法与Flow800软件分析结果进行一致性检验。Kappa值≤0.2为极低的一致性,>0.2~0.4为一般的一致性,>0.4~0.6为中等的一致性,>0.6~0.8为高度的一致性,>0.8~1.0为几乎完全一致。结果(1)20个术前视频中,HS光流法共有18个视频测量结果与Flow800软件分析结果一致,Kappa值为0.780,P<0.01。(2)20个术后视频中,HS光流法共有17个视频测量结果与Flow800软件分析结果一致,Kappa值为0.760,P<0.01。(3)HS光流法和Flow800软件对受体血管改变血流方向判读的一致性结果显示,20例患者中,Flow800软件分析经手术后有11例患者的受体血管改变了血流方向,HS光流法显示有9例,Kappa值为0.700,P=0.002。结论HS光流法结合吲哚氰绿视频可有效辅助术中受体血管血流方向的判断。 Objective To explore the application value of Horn-Schunck(HS)optical flow method combined with indocyanine green intraoperative angiography in cerebral blood flow reconstruction.Methods From December 2018 to June 2019,20 patients with Moyamoya disease was retrospectively included in the Department of Neurosurgery,Huashan Hospital,Fudan University.All of them underwent superficial temporal artery-middle cerebral artery bypass grafting using surgical microscope with built-in Flow800 software.Their indocyanina green(ICG)angiography videos before and after the operation were analyzed using HS optical flow method.By calculating the optical flow vector of each pixel in the receptor vessel,the flow direction of the receptor vessel was semi-quantified.A total of 40 videos of the 20 patients were interpreted by Flow800 software.If the optical flow method and Flow800 software are consistent in the interpretation of the flow direction(forward,reverse)in the preoperative video and the flow direction(forward,reverse,middle to both sides)in the postoperative video,it is considered correct.Each patient was judged whether the blood flow direction changed after operation.If both the optical flow method and Flow800 software showed that the direction was changed,it was considered to be consistent.The consistency between HS optical flow method and Flow800 software was tested.Kappa value≤0.2 is very low consistency,>0.2-0.4 is general consistency,>0.4-0.6 is medium consistency,>0.6-0.8 is high consistency,and>0.8-1.0 is almost complete consistency.Results(1)Among the 20 preoperative videos,18 videos with the HS optical flow method were consistent with the results of Flow800 software,and the Kappa value was 0.780,P<0.01.(2)Among the 20 postoperative videos,17 of 20 videos were consistent with Flow800 software,and the Kappa value was 0.760,P<0.01.(3)The consistency of HS optical flow method and Flow800 software in the interpretation of change of blood flow direction in the recipient vessel showed among 20 patients,11 patients had ch
