摘要
目的探究高血压患者下肢术行外周神经阻滞麻醉与传统腰硬联合麻醉对其术后应激反应的影响作用。方法方便选取该院2018年2月—2019年2月接收的高血压行下肢手术患者80例为研究对象,按照完全随机方法,均分成两组。对照组患者全部接受传统腰硬联合麻醉方法,研究组患者全部接受外周神经阻滞麻醉方法,分别就两组患者的心率、血压、肾上腺素水平以及不良反应发生率实施对比。结果研究组患者术中15 min的心率水平为(71.2±6.5)次/min,收缩压和舒张压分别是(135.6±29.3)mmHg和(71.3±6.8)mmHg,明显优于对照组患者15 min的心率水平(61.4±8.2)次/min,收缩压(117.4±20.6)mmHg和舒张压(60.2±7.5)mmHg;研究组不良发生率40.0%优于对照组70.0%,差异统计学有意义(χ2=7.272,P<0.05)。结论高血压患者下肢术行外周神经阻滞麻醉,可有效改善患者血流动力学情况,避免多种术后不良反应与应激反应,具有较高的临床应用价值。
Objective To explore the effect of peripheral nerve block anesthesia combined with traditional lumbar and hard anesthesia on the postoperative stress response in patients with hypertension.Methods Eighty patients with hypertension who underwent lower extremity surgery received in the hospital from February 2018 to February 2019 were convenient selected as the study object.They were divided into two groups according to the completely random method.The patients in the control group all received traditional combined lumbar and hard anesthesia,and the patients in the study group all received peripheral nerve block anesthesia methods.The heart rate、blood pressure、The levels of norepinephrine(NE),epinephrine(E)and incidence rates were compared between the two groups.Results The heart rate of the patients in the study group at 15 min during the operation was(71.2±6.5)beats/min,and the systolic and diastolic pressures were(135.6±29.3)mmHg and(71.3±6.8)mmHg,respectively,which was significantly better than that of the control group at 15 minutes of(61.4±8.2)times/min,systolic blood pressure(117.4±20.6)mmHg and diastolic blood pressure(60.2±7.5)mmHg;the incidence of adverse events in the study group was 40.0%better than the control group 70.0%,the difference was statistically significant(χ2=7.272,P<0.05).Conclusion Peripheral nerve block anesthesia for lower extremity surgery in patients with hypertension can effectively improve the patient's hemodynamics,avoid various postoperative adverse reactions and stress reactions,and has high clinical application value.
