摘要
目的探讨超声弹性成像对乳腺癌的诊断价值及与ER、PR表达的相关性。方法随机选取乳腺癌患者90例,均接受超声弹性成像检查,应用免疫组化法测定ER、PR,然后分析90例患者ER、PR免疫组化测定结果、超声弹性成像特征与ER、PR表达的相关性。结果90例患者ER阳性60例,阴性30例,阳性率为66.7%(60/90);PR阳性66例,阴性24例,阳性率为73.3%(66/90)。90例患者中,肿瘤直径<3.0 cm 36例,≥3.0 cm 54例,有强回声晕46例,有微钙化38例,有边缘毛刺征55例,后方衰减56例,形状规则44例,周界清晰37例,有淋巴结转移44例,弹性评分1~3级38例,4~5级52例。有边缘毛刺征患者的ER、PR阳性率显著高于无边缘毛刺征患者(P<0.05),有后方衰减患者的ER、PR阳性率显著高于无后方衰竭患者(P<0.05),形状不规则患者的ER、PR阳性率显著高于形状规则患者(P<0.05),边界不清晰患者的ER、PR阳性率显著高于边界清晰患者(P<0.05),有淋巴结转移患者的ER、PR阳性率显著高于无淋巴结转移患者(P<0.05),弹性评分4~5级患者的ER、PR阳性率显著高于弹性评分1~3级患者(P<0.05),但不同肿瘤直径、强回声晕、微钙化患者的ER、PR阳性率之间的差异均不显著(P>0.05)。结论超声弹性成像技术在诊断乳腺癌方面价值较高,能够为临床早期诊断乳腺癌提供影像学依据,而乳腺癌患者中ER、PR表达对于患者术后治疗及预后评估方面为临床提供帮助,因此两者联合应用对于乳腺癌的术前诊断和术后评估患者预后有较高的临床价值,值得在临床推广应用。
Objective To explore the diagnostic value of ultrasound elastography in breast cancer and its correlation with the expression of ER and PR.Methods 90 cases of breast cancer were randomly selected,all of them were examined by ultrasound elastic imaging,ER and PR were measured by immunohistochemistry,and then the correlation between ER and PR expression and the characteristics of ultrasound elastic imaging was analyzed.Results There were 60 ER positive cases and 30 negative cases in 90 patients,the positive rate was 66.7%(60/90),66 PR positive cases and 24 PR negative cases,the positive rate was 73.3%(66/90).Among 90 patients,36 had tumors<3.0 cm in diameter and 54 had tumors(>3.0 cm)in diameter,46 had strong echo halo,38 had microcalcification,55 had marginal burr sign,56 had posterior attenuation,44 had regular shape,37 had clear periphery and 44 had lymph node metastasis.38 cases had elastic score of Grade 1-3 and 52 cases had elastic score of grade 4-5.The positive rates of ER and PR in patients with marginal burr sign were significantly higher than those without marginal burr sign(P<0.05),those with posterior attenuation were significantly higher than those without posterior failure(P<0.05),and those with irregular shape were significantly higher than those with regular shape(P<0.05).The positive rate of ER and PR in patients with lymph node metastasis was significantly higher than that in patients without lymph node metastasis(P<0.05).The positive rate of ER and PR in patients with elasticity score 4-5 was significantly higher than that in patients with elasticity score 1-3(P<0.05).However,the patients with different tumor diameter,strong echo halo and microcalcification had higher ER and PR positive rates than those without lymph node metastasis(P<0.05).There was no significant difference between ER and PR positive rates(P>0.05).Conclusion Ultrasound elastography is of high value in the diagnosis of breast cancer,and it can provide imaging basis for early diagnosis of breast cancer.The expression of ER and P
