摘要
目的探讨多普勒超声在早期乳腺癌筛查中的应用价值及超声参数与患者病理的相关性。方法选取2016年3月至2019年1月间陕西省宝鸡市中心医院收治的68例早期乳腺癌患者,均采用多普勒超声检查。以病理学检测结果为金标准,计算多普勒超声对早期乳腺癌检测的敏感性、特异性及准确性,分析多普勒超声指标与病理的相关性。结果多普勒超声对早期乳腺癌筛查一致性为88.2%,灵敏度为90.6%,特异度为50.0%。不同形态特征肿块的肿瘤浸润状态比较,差异有统计学意义(P <0.05)。肿块形态特征评估早期乳腺癌的一致性为79.4%,灵敏度为82.8%,特异度为25.0%。不同边缘特征肿块的肿瘤浸润状态比较,差异有统计学意义(P <0.05)。通过肿块边缘特征评估早期乳腺癌一致性为79.4%,灵敏度为82.8%,特异度为25.0%。不同边界特征肿块的肿瘤浸润状态比较,差异有统计学意义(P <0.05)。肿块边界特征评估早期乳腺癌一致性为85.3%,灵敏度为85.9%,特异度为75.0%。结论多普勒超声检测诊断早期乳腺癌有较好的诊断筛查价值,超声特征变化也与早期乳腺癌病理具有一定相关性,是一种较便捷的早期乳腺癌筛查手段。
Objective To explore the value of Doppler ultrasound in breast cancer early screening and the correlation between ultrasound parameters and the pathology in breast cancer patients. Methods Sixty-eight patients with suspected early-stage breast cancer who visited Baoji Central Hospital from March2016 to January 2019 were selected as the research subjects. Doppler ultrasonography was performed on them,and the pathological test results were used as the gold standard. The sensitivity,specificity and accuracy of Doppler ultrasound for the detection of early-stage breast cancer was estimated. At the same time,the correlation of Doppler ultrasound indicators such as tumor morphology,tumor edge,and tumor boundary with pathological test results were analyzed. Results The calculated consistency was 88. 2%,the sensitivity was 90. 6%,and the specificity was 50. 0% of Doppler ultrasound for breast cancer early screening.There were significant differences in tumor infiltrating state among lumps with different morphological characteristics( P < 0. 05). The consistency of different morphological characteristics for early-stage breast cancer was 79. 4%,the sensitivity was 82. 8% and the specificity was 25. 0%. There were significant differences in in tumor infiltrating state among lumps with different edge characteristics for early-stage breast cancer. The consistency was 79. 4%,the sensitivity was 82. 8% and the specificity was 25. 0%. There were significant differences in tumor infiltrating state among lumps with different boundary characteristics.The consistency was 85. 3%,the sensitivity was 85. 9%( 55/64),and the specificity was 75. 0%. Conclusion Doppler ultrasound has good diagnostic and screening value for early breast cancer. At the same time,the changes in ultrasound characteristics are also related to the pathology of early breast cancer. It is a convenient method for breast cancer early screening.
