摘要
检索国内外主要期刊数据库中有关中西医结合疗法(治疗组)对比单纯西药(对照组)治疗绝经过渡期功能失调性子宫出血的临床随机对照试验,采用Cochrane手册中的偏倚风险评估工具评价其文献质量,采用RevMan 5.3统计软件进行Meta分析,以评价其疗效和有效性。结果共纳入33篇临床随机对照试验文献。Meta分析结果显示,治疗组的总有效率[OR=3.95,95%CI=(3.05,5.11)]与中医证候有效率[OR=5.36,95%CI=(2.79,10.29)]及其在改善血红蛋白水平[MD=9.99,95%CI=(6.56,13.42)],降低不良反应率[OR=0.30,95%CI=(0.11,0.83)],以及改善复发[OR=0.28,95%CI=(0.19,0.41)]方面明显优于对照组,差异具有统计学意义(P<0.05)。结果提示中西医结合治疗绝经过渡期功能失调性子宫出血较单纯西医治疗具有一定的优势,且具有较好的安全性。
The clinical randomized controlled trials of integrated Chinese and western medicine therapy(treatment group)vs western medicine(control group)for the treatment of abnormal uterine bleeding during the menopausal transition in the main domestic and oversea databases were reviewed. Tools for analysis of bias recommended by Cochrane Reviewer’s Handbook was used for the evaluation of the quality of the literatures,and RevMan 5.3 was adopted for the data extract, and meta-analysis. And then the clinical effictiveness and safety of integrated Chinese and western medicine for the treatment of abnormal uterine bleeding during the menopausal transition were systematically reviewed. The results showed that a total of 33 randomized controlled trials were included, and involved 1190 cases. The results of meta-analysis showed that the treatment group had higher total effective rate[OR = 3.95,95%CI =(3.05,5.11)] and effective rate for traditional Chinese medicine syndrome [OR = 5.36,95%CI =(2.79,10.29)],and had stronger effects than the control group on increasing the hemoglobin level[MD = 9.99, 95% CI =(6.56, 13.42)], reducing adverse reaction incidence [OR = 0.30, 95% CI =(0.11,0.83)], and improving the relapse [OR = 0.28, 95% CI =(0.19, 0.41)], the differences being statistically significant(P<0.05). The analysis results indicated that the integrated Chinese and western medicine exerts certain clinical efficacy and safety for the treatment of abnormal uterine bleeding during the menopausal transition.
