摘要
【目的】探讨中医规范化治疗突发性聋的疗效影响因素。【方法】对791例(791耳)采用中医规范化治疗的突发性聋患者的临床资料进行回顾性分析,统计患者性别、年龄、耳别、病程、耳聋程度、听力曲线类型、伴随症状(耳鸣、耳堵、眩晕)和既往治疗史对疗效的影响。【结果】(1)总体疗效方面:791例患者中,痊愈179例,显效115例,有效141例,无效356例,痊愈率为22.63%,总有效率为54.99%。(2)疗效的影响因素方面:老年患者的疗效较中青年患者差;病程越短则疗效越好;耳聋程度越轻则痊愈的可能性越大;听力曲线为低频下降型、平坦型、高频下降型、全聋型者,其痊愈率依次降低;伴有眩晕者的痊愈率较无眩晕者低;既往经过西医治疗无效患者的疗效较未经过任何治疗患者差,差异均有统计学意义(P<0.05或P<0.01)。【结论】中医规范化治疗突发性聋具有一定的疗效,而患者的年龄、病程、耳聋程度、听力曲线类型、是否伴眩晕、既往有无治疗史等因素对疗效有不同程度的影响;部分经西医常规治疗无效的患者,运用中医综合治疗仍有一定的疗效。
Objective To investigate the curative effect influencing factors of sudden deafness treated by standardized comprehensive treatment of traditional Chinese medicine(TCM). Methods A retrospective analysis was made on the clinical data of 791 cases(involving 791 ears)of patients with sudden deafness who received standardized comprehensive treatment of TCM. The influences of gender,age,left or right ear,the course of disease,severity of hearing loss,hearing curve types,concurrent symptoms such as tinnitus,aural fullness and vertigo,and the medication history on the curative effect were investigated. Results(1)In respect of the overall efficacy, 179 of the 791 cases were cured, 115 were markedly effective, 141 were effective, and 356 were ineffective, with the cure rate being 22.63% and the total effective rate being 54.99%.(2)In respect of the curative effect influencing factors,curative effect for the elderly patients was superior to that for the young and middle-aged patients, better curative effect was achieved in the patients with the shorter course of disease,patients with less severity of the deafness had the greater possibility of being cured,and the cure rate was in the decreasing sequence in the patients with hearing curve types being low-frequency descending, flat, highfrequency descending, and totally deaf. The patients accompanied with vertigo had lower cure rate than the patients without the complication of vertigo, and the patients being ineffective after the medication of western medicine had weaker curative effect than the patients had no medication history. The differences of the above influencing factors were statistically significant(P<0.05 or P<0.01). Conclusion Standardized comprehensive treatment of TCM has certain efficacy for the treatment of sudden deafness. Factors affecting the outcome include age, the course of disease, severity of hearing loss, hearing curve types, presence of vertigo, and the medication history.
