摘要
目的:评价抗VEGF药物与Ahmed青光眼引流阀植入术联合治疗新生血管性青光眼的临床效果。方法:回顾性分析洛阳市第一人民医院2017年1月至2019年2月收治的新生血管性青光眼80例(80眼)的临床资料。患者分为两组,对照组接受Ahmed引流阀植入术,研究组接受玻璃体内注射康柏西普联合Ahmed引流阀植入术。术后随访3个月,比较两组治疗效果。结果:研究组术后眼压、最佳矫正视力均优于对照组(t=6.165,3.508;P=0.000,0.001)。研究组术后抗青光眼药物使用种类为(0.6±0.2)种,少于对照组的(1.1±0.5)种(t=5.8722;P=0.000)。研究组术后1天和3天的手术成功率均为97.5%(39/40),术后1周为95.0%(38/40),术后1个月为92.5%(37/40),术后3个月为90.0%(36/40),均高于同时间点对照组(P<0.05)。研究组并发症发生率为5.0%(2/40)低于对照组的25.0%(10/40)(χ^2=6.275,P=0.012)。结论:联合应用抗VEGF药物与Ahmed引流阀治疗新生血管性青光眼,可有效降低眼压,改善视力,提高手术成功率,且并发症少。
Objective To evaluate the clinical efficacy of anti-vascular endothelial growth factor(anti-VEGF)drug combined with implantation of Ahmed glaucoma drainage valve for the treatment of neovascular glaucoma.Methods The clinical data of 80 eyes of 80 cases with neovascular glaucoma admitted to the First People’s Hospital of Luoyang from Jan.2017 to Feb.2019 were retrospectively analyzed.The patients were divided into two groups.The control group received Ahmed drainage valve implantation and the study group received conbercept intravitreal injection combined with Ahmed drainage valve implantation.The efficacy of the two groups were compared.The follow-up time was 3 months.Results The postoperative IOP and BCVA of the study group were better than those of the control group(t=6.165,3.508;P=0.000,0.001).The kinds of postoperative antiglaucoma drugs used in the study group were(0.6±0.2)types,less than(1.1±0.5)types in the control group(t=5.872,P=0.000).The success rate of the study group was 97.5%(39/40)at 1 and 3 days after surgery,95.0%(39/40)at 1 week after surgery,92.5%(37/40)at 1 month after surgery,and 90.0%(36/40)at 3 months after surgery,the difference of the success rate in the study group was higher than that of the control group at the same time(P<0.05).The incidence of complications in the study group was 5.0%(2/40)lower than that in the control group 25.0%(10/40)(χ2=6.275,P=0.012).Conclusion Anti-VEGF drug combined with implantation of Ahmed drainage valve in the treatment of neovascular glaucoma can effectively reduce the intraocular pressure,improve the visual acuity,and improve the success rate of surgery,with fewer complications.
