Correlation of Onset Time of Acute Exacerbation of Chronic Obstructive Pulmonary Disease with the Twenty-four Solar Terms

摘要 【目的】探讨慢性阻塞性肺疾病急性加重期(AECOPD)发病时间与二十四节气相关性,以期为临床AECOPD的防治提供参考。【方法】选择2015年4月5日~2019年4月4日广东省中西医结合医院信息管理系统(HIS)数据库中呼吸内科收治入院的长期居住于佛山市南海区的慢性阻塞性肺疾病病程为10年以上,且平素规律用药的AECOPD病例,通过调查其发病日期,明确发病节气,采用频数分布和圆形分布统计方法,分析AECOPD发病时间与二十四节气的相关性。【结果】165例AECOPD患者在二十四节气均可发病,未呈现出特定的发病高峰,相对高发的散在节气主要分布在立春(15例,占9.09%)、惊蛰(12例,占7.27%)、夏至(10例,占6.06%)、芒种(9例,占5.45%)、霜降(8例,占4.85%)和立冬(8例,占4.85%)。按季节来分,则以春季(30.91%)、冬季(25.45%)发病为主,高于夏季和秋季的21.82%。相对高峰期为冬至至白露(相当于上一年的12月22日至9月8日)。【结论】AECOPD的发病可能与节气相关,冬至至白露为其相对高发节气区间。 Objective To explore the correlation of the onset time of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)with the 24 solar terms,so as to provide reference for the clinical prevention and treatment of AECOPD. Methods The study enrolled the cases of chronic obstructive pulmonary disease(COPD)which were admitted in the Respiratory Medicine Department of Guangdong Provincial Hospital of Integrated Chinese and Western Medicine from April 5,2015 to April 4,2019. The enrolled cases of COPD with the course of disease over 10 years and with long-term dwelling in Nanhai district of Foshan city were medicated regularly throughout the course of the disease. The information of the COPD cases were collected from the database of Hospital Information System(HIS)of Guangdong Provincial Hospital of Integrated Chinese and Western Medicine,which included the onset date and the solar terms for the onset of COPD. The correlation of the onset time of AECOPD with the 24 solar terms was investigated by the statistical methods of frequency distribution and circular distribution. Results The onset of 165 AECOPD patients occurred at various solar terms, and the onset of AECOPD did not show a specific peak. The relatively high incidence of AECOPD occurred in the Beginning of Spring(15 cases, accounting for 9.09%), the Waking of Insects(12 cases, accounting for 7.27%), the Summer Solstice(10 cases,accounting for 6.06%),Grain in Beard(9 cases,accounting for 5.45%),Frost’s Descent(8 cases,accounting for 4.85%),and the Beginning of Winter(8 cases,accounting for 4.85%). In respect of the onset season,30.91% of AECOPD occurred in the spring,25.45% of AECOPD occurred in the winter,and 21.82% of AECOPD occurred in the summer and in the autumn. The relative peak period is from the Winter Solstice to White Dew(equivalent to December 22 of the previous year to September 8).
