序贯机械通气在重症肺炎呼吸衰竭患者中的应用效果研究 被引量:2

摘要 目的探究重症肺炎呼吸衰竭患者应用序贯机械通气的临床应用效果。方法抽取该院2015年6月—2019年12月48例重症肺炎呼吸衰竭患者,给予随机分组,将给予传统机械通气与序贯机械通气治疗的患者分别选为常规组(24例)与研究组(24例)。对患者的临床治疗时间、血流动力学指标变化予以对比。结果常规组总通气时间(13.53±1.58)d、有创通气时间(13.25±1.46)d及住院时间(21.89±2.14)d,与研究组各治疗时间相比明显更长(P<0.05)。两组治疗前的各血流动力学指标差异无统计学意义(t=0.530、0.670、0.470、0.960,P>0.05);两组进入PIC时的氧分压与二氧化碳分压对比,差异无统计学意义(t=0.820、1.110,P>0.05);常规组患者进入PIC时心率(98.63±8.45)次/min、呼吸频率(24.84±2.82)次/min,与研究组差异有统计学意义(t=5.360、9.650,P<0.05)。结论序贯机械通气在重症肺炎呼吸衰竭患者中应用效果优于传统机械通气治疗,缩短了治疗时间,减少了患者的病死与再次插管情况,对患者生活质量改善作用较为显著。 Objective To explore the clinical application of sequential mechanical ventilation in patients with severe pneumonia and respiratory failure.Methods A total of 48 patients with severe pneumonia and respiratory failure from June 2015 to December 2019 in the hospital were randomly assigned.The patients given traditional mechanical ventilation and sequential mechanical ventilation were selected as the conventional group(24 cases)and the research group(24 cases).The clinical treatment time and hemodynamic index changes of patients were compared.Results The total ventilation time(13.53±1.58)d,invasive ventilation time(13.25±1.46)d and hospital stay(21.89±2.14)d in the conventional group were significantly longer than those in the study group(P<0.05).There was no statistically significant difference in the hemodynamic indexes between the two groups before treatment(t=0.530,0.670,0.470,0.960,P>0.05);the oxygen partial pressure and carbon dioxide partial pressure when the two groups entered PIC,,the difference was not statistically significant(t=0.820、1.110,P>0.05);the heart rate(98.63±8.45)beats/min and respiratory rate(24.84±2.82)beats/min of patients in the conventional group when entering PIC were statistically significantly different from the study group(t=5.360,9.650,P<0.05).Conclusion Sequential mechanical ventilation is better than traditional mechanical ventilation in patients with severe pneumonia and respiratory failure,which shortens the treatment time,reduces the patient's death and reintubation,and has a significant effect on improving the quality of life of patients.
