光化学法诱导兔视网膜分支静脉阻塞模型的初步探讨

摘要 目的探讨采用光化学法建立兔视网膜分支静脉阻塞(BRVO)模型的方法,观察模型的自然病程及造模后的病理改变。方法健康成年新西兰兔共40只,分为3组,第1组8只进行连续观测,第2组24只,每个时间点6只分别进行单点观察,第3组8只为正常对照组。第1、2组均右眼造模,经兔耳缘静脉推注10%荧光素钠注射液(0.3 ml/kg)后,右眼在激光接触镜下,以能量100~300 mW,光斑直径100~300μm,波长530 nm,持续时间0.2~0.5 s,应用532激光距离视盘0.5个视盘直径(DD)外进行节段性光凝,光凝一侧或双侧分支静脉,每一静脉击射平均约30点。造模前后均行眼底彩色照相和荧光素眼底血管造影(FFA)检查,并分别在7、14、21和28 d行视网膜组织病理学检查。结果(1)眼底彩照和FFA检查证实2组兔眼造模后均有BRVO表现,并且2组实验兔光凝血管阻塞程度表现大致相同,模型维持时间及血管再通时间也基本一致,个体差异中循环障碍维持时间各有不同,最长21 d,最短7 d,约81%能维持14~21 d。(2)组织病理学检查示在14 d时,视网膜神经节细胞数:模型兔(2.83±1.17)个明显少于正常兔(12.33±8.07)个,t=2.480,P=0.02,差异有统计学意义;外核层细胞层数:模型兔(4.00±1.27)层少于正常对照组(5.83±0.75)层,t=4.166,P=0.000,有统计学意义。在其他时间点模型组基本同正常组。(3)总体上14 d前模型兔眼视网膜形态结构改变、血栓形成及循环障碍性改变符合BRVO临床病理情况,21 d后逐渐恢复正常。结论兔光化学法可建立与临床病理机制相似的、可靠的BRVO动物模型,造模操作方便,方法易于掌握和眼底观察。该模型的维持时间及病程演变,为BRVO的临床与基础研究提供了必要参考。 OBJECTIVE To explore the photochemical method on establishment of branch retinal vein occlusion model in rabbits,and to observe the natural course of the disease and pathological changes after modeling.METHODS A total of 40 healthy adult New Zealand rabbits were divided into 3 groups.Eight rabbits in the first group for continuous observation;24 rabbits in the second group,and 6 of them observed in each time point;8 rabbits in the third group as the normal control group.Rabbits of the first and the second group were modeled in the right eye.Under the laser contact lens,the right eyes were photocoagulated with laser of 100~300 mw,spot diameter of 100~300μm,wavelength of 530 nm,and duration of 0.2~0.5 s after bolus injection of 10%sodium fluorescein injection(0.3ml/kg)through the ear vein of rabbit.Segmenting photocoagulation was performed with a 532-type laser at a distance of 0.5 disc diameter from the optic disc,with one or both branches of the vein photocoagulation,and each vein was shot at an average of about 30 points.Fundus photograph and fundus fluorescein angiography(FFA)was performed before and after the modeling then the retinal histopathological examination was performed on the 7th,14th,21st and 28th day.RESULTS Fundus photograph and FFA confirmed the presence of BRVO in the two groups after modelling.In addition,the degree of obstruction of photocoagulated blood vessels in the two groups of experimental rabbits was roughly the same,the maintenance time of the model,and the recanalization time of the blood vessels were also basically the same.The maintenance time of circulatory disorders in individual differences was different,with the longest 21 days and the shortest 7 days,and about 81%could maintain 14 to 21 days.Histopathological examination showed the number of retinal ganglion cells in the model rabbits(2.83±1.17)was significantly less than that in the normal rabbits(12.33±8.07)on the 14th day,with statistical significance(t=2.480,P=0.02);at the same time,the number of cells in the outer nuclea
